Fetomaternal hemorrhage with intraplacental chorioangioma

Abstract: A 37-year-old Asian woman, gravid 0 para 0, was admitted to our hospital at 34 weeks and 5 days of her pregnancy for management of preeclampsia. A few days after admission, she recognized diminished fetal movement, and a non-stress test revealed a non-reassuring fetal heart rate pattern with decreased variability. A female baby weighing 1840 g was delivered by emergency cesarean section with Apgar scores of 5 and 5 at 1 and 5 min, respectively. Significant neonatal anemia with a hemoglobin level of 4.3 g/dL wa… Show more

“…16 Fetomaternal hemorrhage can be diagnosed by demonstrating the elevated level of fetal hemoglobin in the maternal circulation. 17 Serum and/or amniotic fluid levels of alpha-fetoprotein (AFP) can be increased in cases of placental angioma. Increased serum levels of AFP is not pathognomonic for placental angioma since AFP is increased physiologically during pregnancy and a pathological increase of its level may attributed to other fetal and maternal medical conditions.…”

“…The risk factors causing FMH include blood type incompatibility, abdominal trauma, amniocentesis, external cephalic inversion, hypertensive disorders or placental and umbilical cord abnormalities, such as choriocarcinoma [4,5]. However, the two rare cases reported here are idiopathic in origin, spontaneous, and uncomplicated near-term pregnancies.…”

“…Los factores predisponentes que se han postulado incluyen preeclampsia 15 , procedimientos diagnósticos invasivos como amniocentesis, cordocentesis y biopsia de vellosidades coriales 16,17 , trauma abdominal [18][19][20] , tumores placentarios 15 , y maniobras como versión cefálica externa 21,22 . Sin embargo, más del 80% de los casos aparecen como de etiología incierta, lo cual dificulta su diagnóstico e intervención oportunos.…”
