Fibromyalgia: Recent Advances in Diagnosis, Classification, Pharmacotherapy and Alternative Remedies

Abstract: Fibromyalgia (FM) is a syndrome that does not present a well-defined underlying organic disease. FM is a condition which has been associated with diseases such as infections, diabetes, psychiatric or neurological disorders, rheumatic pathologies, and is a disorder that rather than diagnosis of exclusion requires positive diagnosis. A multidimensional approach is required for the management of FM, including pain management, pharmacological therapies, behavioral therapy, patient education, and exercise. The purp… Show more

“…FM is a disease that results from the impaired neurotransmission mediated by serotonin and catecholamines [ 40 , 41 , 42 ]. This altered neurotransmission underlies many symptoms of this disease, including chronic pain, mood and sleep disorders, and cognitive decline [ 43 ]. In FM dysregulated dopaminergic neurotransmission was shown and a strong correlation between dopamine metabolism and reduced gray matter density was reported.…”

“…Fibromyalgia (FM) is a syndrome characterized by chronic pain with multiple tender points, increased pain sensitivity, and other systemic symptoms like cognitive dysfunction, sleep disturbance, anxiety, chronic fatigue, and depression, in the absence of any well-defined underlying organic disease [ 9 ]. In most cases, the pain is not explained by inflammation or injury.…”

“…Lower levels of 5-HTP were found in women with fibromyalgia (FM) in comparison with controls, indicating that the dysregulation of the catecholamine and indolamine pathway in patients with FM may contribute to the physiopathology of this syndrome [ 165 , 166 ]. The treatment with 5-HTP significantly improved all the clinical parameters studied in 50 patients with primary FM syndrome, with only mild and transient side effects reported [ 167 ].…”
