“…Cytokeratin and AFP levels are not useful; hence, an acinar arrangement and EMH are the main features apart from the finer morphological details of embryonal cells described previously. The presence of bizarre giant cells is very rare and is described more commonly in undifferentiated embryonal sarcoma of the liver (a tumor with a mesenchymal morphology that also shows mucoid material and tumor cell spindling) or HCC . The small cell undifferentiated type of HB is diagnosed if cytokeratin is positive, AFP is low, and SALL4 is negative; this contrasts with embryonal cells, which are also cytokeratin positive but show an AFP elevation, an acinar arrangement, and strong SALL4 positivity.…”
