Fine‐needle aspiration of lipoblastoma: Cytological, molecular, and clinical features

Abstract: FNA, especially if used together with molecular biology techniques (eg, PLAG1 FISH analysis), is a reliable and accurate diagnostic tool. Cancer Cytopathol 2017;125:934-9. © 2017 American Cancer Society.

“…Ultrasonography-guided FNA is a minimally invasive sampling technique. Many papers including case reports have shown its safety, clinical usefulness, and limitation for diagnosing retroperitoneal tumors such as schwannoma [25], lipoblastoma [26], histiocytic sarcoma [27], liposarcoma [28], and abscess. Most of them are not able to be diagnosed solely from image findings.…”

“…However, diagnostic accuracy of endoscopic ultrasound-guided FNA followed by cytomorphologic features and immunocytochemistry analysis is only 66.7% in schwannoma [25]. Out of nine patients with lipoblastoma, the FNA diagnosis was inconclusive due to hypocellularity in one patient, and a diagnosis of benign lipomatous tumor was made in another patient [26]. Regarding histiocytic sarcoma, the diagnosis with FNA alone is extremely challenging [27].…”

“…Both the tumours were of large size. Previous reports of FNA of lipoblastoma have also highlighted similar myxoid change and delicate capillary network as important morphological features . By contrast, the presence of uni‐/multi‐vacuolated lipoblasts can lead to an over‐diagnosis of myxoid liposarcoma …”

“…Microscopic features consist of lobular architecture and myxoid areas with spindle cells and lipoblasts at various stages of differentiation [28,29].…”

“…Rearrangements of chromosome 8q11 codifying for the oncogene PLAG1 are found in more than 70% of cases [2][3][4][28][29][30]. The different molecular alteration found in adipocytic tumors could help in differentiating between lipoma, lipoblastoma, and liposarcoma [31].…”
