Assessment of Circulating Microribonucleic Acids in Patients With Familial Mediterranean Fever

Objectives: This study aims to evaluate the plasma expression of microribonucleic acids (miRNAs) that may be associated with the pathogenesis of familial Mediterranean fever (FMF). Patients and methods: Thirty patients with FMF (18 males, 12 females; mean age 9.1±4.7 years; range, 3 to 15.5 years) and 30 age-and sex-matched healthy children (18 males, 12 females; mean age 9.5±4.6 years; range, 4 to 16.5 years) were included in this study. The plasma levels of four candidate miRNAs (miRNA-16, miRNA-155, miRNA-204 and miRNA-451) were measured in all subjects. The plasma levels of miRNAs were analyzed with real-time polymerase chain reaction in attack and remission periods of patients and healthy controls (HCs). Results: Plasma miRNA-204 levels of FMF patients were decreased 6.5 fold in remission period compared to HCs (p<0.001). This decrease was more prominent in M694V mutation carriers. Plasma miRNA-155 levels of FMF patients were lower in remission period (p=0.03). Conclusion: Our findings showed significant alterations in the plasma expression of miRNA-155 and miRNA-204 in FMF patients compared to HCs. Our data suggest that miRNA-155 and miRNA-204 may be related to the pathogenesis of FMF. Further comprehensive and functional researches may help to clarify the role of miRNAs in FMF and elucidate the pathogenesis of the disease.

Inflammation causes debilitating human conditions and older treatments rely on global immunosuppression that non-specifically alleviates symptoms. Currently, several monoclonal antibodies (mAbs) are available that specifically block pro-inflammatory cytokines. These include mAbs specific to tumour necrosis factor (TNF), interleukin (IL)-1β,I L -6 , IL-17 and IL-12/IL-23. The chapter summarises the key elements in human inflammatory disease conditions, including various forms of arthritis, psoriasis, Crohn's disease and ulcerative colitis, plus pyrin-associated inflammatory syndromes and periodic fevers, to explain the benefit of cytokine neutralisation through mAb-type reagents. The chapter reviews the efficacy and safety of the current repertoire of anti-cytokine/cytokine receptor mAbs. It also discusses the known side effects and adverse events that are sometimes associated with systemic blockade of cytokines in vivo, and concludes that the accumulating knowledge of treatment failures can reveal unappreciated aspects of cytokine biology and even new treatment opportunities. The chapter includes mention of the rapidly expanding cohort of biosimilar mAbs and the mAbs to IL-4, IL-5 and IL-13 that are now emerging, in addition to the need for treatments for disorders that remain refractory to the current repertoire of anti-cytokine mAbs and conventional treatments. Thus, here we summarise the current status of anti-cytokine mAbs for human inflammatory diseases.

Ricevuto: 16 marzo 2015 / Accettato: 2 aprile 2015 © Springer-Verlag Italia 2015Riassunto Il microarray, spesso paragonato alla polymerase chain reaction e alle tecniche del DNA ricombinante, è tra le metodiche di biologia molecolare quella di più forte impatto scientifico. Nonostante la tecnica si basi su principi noti della biochimica e della biologia molecolare, come la specificità di legame di sequenze nucleotidiche complementari, a un certo momento del processo evolutivo tecnologico è stato possibile effettuare analisi su grande scala avvalendosi di tecniche miniaturizzate; è stata questa la vera conquista che ha portato gli scienziati a fare un grande "balzo avanti". Esistono varie classi di microarray, tutte di grande interesse sia nel campo della ricerca di base sia nella diagnostica medica. In certi settori della medicina la tecnica è entrata nella diagnostica di routine, mentre in altri rappresenta solo una speranza in attesa di validazione. Esistono problematiche diverse per ciascuna tipologia di microarray; in generale, il disegno sperimentale, l'elaborazione del campione, la normalizzazione dei dati e infine l'analisi e l'interpretazione costituiscono i passaggi fondamentali per produrre risultati con tecnologia microarray. Oggi le tecniche di biologia molecolare permettono l'analisi contemporanea di migliaia di geni, proteine o metaboliti e hanno cambiato la dimensione della ricerca biologica, facendo passare i ricercatori dallo studio dei singoli geni o proteine allo studio sistematico di tutti i geni e proteine. Risulta, quindi, di particolare interesse per la medicina molecolare poter disporre di una visione globale dei processi biologici, aprendo così la strada per migliaia di potenziali applicazioni.
