Five adult laryngeal venous malformation cases treated effectively with sclerotherapy

Abstract: Laryngeal venous malformation is a comparatively rare condition in adults. It presents as a livid or bluish mass that may cause bleeding, hoarseness, stridor, and even difficulty in breathing. So far, treatment has largely consisted of laser therapy and surgical removal. Five cases of adult laryngeal venous malformations that were treated by sclerotherapy (local injection of monoethanolamine oleate solution) are reported. All lesions disappeared after one or two treatment sessions, and two of the five cases th… Show more

“…However, there is a paucity of literature concerning multidisciplinary approaches to sclerotherapy and techniques of gaining access to airway lesions. The combination of sclerotherapy with direct laryngoscopy has been recently reported by Kamijo et al in one patient in a series of five adult laryngeal VMs treated with local injection of monoethanolamine oleate solution. The combined technique, in which access to the lesion is gained through direct laryngoscopy or nasopharyngoscopy, allows for optimal exposure in areas that are less likely to be reached by percutaneous sclerotherapy.…”

“…Coblation not only ablates but also coagulates the blood vessels. Lasers are perfect for making incisions; they are poorer for hemostasis and raise concerns about intraoperative bleeding [7]. It is an alternative.…”

“…Sclerotherapy has been used as an adjunct or alternative to surgery. Several sclerosing agents are available including absolute ethanol, ethanolamine oleate, 5 sodium tetradecyl sulfate, 6 polidocanol, 7 bleomycin, 2,8 and OK 432, 9 and all have been reported to be effective. Currently, there is no consensus on the best sclerosing agent.…”
