Five-Fraction Stereotactic Body Radiation Therapy (SBRT) and Chemotherapy for the Local Management of Metastatic Pancreatic Cancer

Abstract: As systemic therapies improve, the local management of pancreatic cancer will become increasingly important. Here, we report significantly improved local control with SBRT of smaller PTV tumors with concurrent chemotherapy. Five-fraction SBRT offers a quick and effective modality of local tumor control with minimal toxicity in the metastatic pancreatic cancer population.

“…Due to the inherent toxic nature of chemotherapy, using a lesser dose of FX therapy reduces toxicity and, in our model, was more effective than full dose chemotherapy. Our preclinical toxicity observations are supported by clinical retrospective studies demonstrating that concurrent administration of SBRT and chemotherapy achieved effective local tumor control with minimal/well tolerated toxicity (52)(53)(54). The concurrent dosing regimen reported here is expected to promote an optimal immune response as SBRT utilizes fewer fractions than conventional radiotherapy meaning that radiosensitive antitumor lymphocytes are likely not destroyed by the repeated radiation fractions given in the conventional schedule.…”

“…There have been few studies investigating the role of SBRT in the local control of primary tumors of metastatic pancreatic cancer. Lischalk et al (24) analyzed 20 patients with pathologically diagnosed metastatic adenocarcinoma of the pancreas. SBRT was conducted on the primary pancreatic tumor in five fractions to a total dose of 25-30 Gy.…”

“…[ 28 , 29 ] Notably, combining an esophagectomy with a pancreaticoduodenectomy requires advanced planning and efficient coordination between both thoracic and hepatobiliary surgeons. A short course of radiation therapy with SBRT may also be a reasonable option to maximize local control with very little toxicity in oligometastatic PDAC, [ 24 , 26 , 27 , 30 ] especially in cases in which a break from systemic therapy is necessary due to intolerability.…”
