Five‐year surveillance of acute flaccid paralysis in Malaysia

Abstract: Overall, these data suggest the absence of circulation of wild poliovirus in Malaysia from 1997 to 2001. The pattern of AFP in this study is different from other published reports.

“…Unlike the incidence target, the adequate stool testing target was only met once by one of the five countries included in this study. This is not uncommon even in regions that consistently meet the incidence target . Given that GBS was the most common diagnosis of non‐polio AFP in each country surveyed, neurological investigations may be favored over stool sampling to make definitive diagnosis in children presenting with AFP.…”

“…This is not uncommon even in regions that consistently meet the incidence target. [14][15][16] Given that GBS was the most common diagnosis of non-polio AFP in each country surveyed, neurological investigations may be favored over stool sampling to make definitive diagnosis in children presenting with AFP. While more sophisticated neurologic imaging modalities would be able to diagnose polio, in a post-elimination era, prior to eradication, stool sampling is an accepted and standard method of obtaining isolates to enable detailed molecular testing that would differentiate between vaccine-derived and wild-type viruses.…”

“…All three PV types may cause inflammation and destruction of anterior horn cells in the spinal cord resulting in acute flaccid paralysis. After WHO launched the global program of eradication of polio in 1988, poliomyelitis due to wild PV is almost eradicated but acute flaccid paralysis associated with non-polio HEVs remains a problem [Chen et al, 2001;Grimwood et al, 2003;Hussain et al, 2004].…”

“…The prognosis of the paralysis caused by EV 71 is unknown. 1,[11][12][13] It may be the same as that of poliomyelitis since the lesions caused by polio virus and the EV 71 virus are similar. Sharrard 14 reported 142 patients with poliomyelitis and examined their muscle recovery in detail.…”
