Flap suture - a simple technique for the revision of hypotony maculopathy following trabeculectomy with mitomycin C

Abstract: Resuturing the scleral flap through the intact conjunctiva is an effective and minimally invasive method to treat hypotony maculopathy following trabeculectomy with MMC. Thus, opening the conjunctiva can be avoided.

“…[14][15][16] More recently, trans-conjunctival scleral flap sutures have been described as a way to control outflow without opening the bleb. 17 The goal is to increase the IOP to a point where the hypotony is resolved without sacrificing the original purpose of the trabeculectomy.…”

“…[14][15][16] Although it is unclear if there is a specific time frame after which the visual changes become permanent, normalization of the IOP should be sought as soon as possible after signs of hypotony are seen. 15,17 In cases in which there is only overfiltration, various techniques are often implemented ranging from conservative to surgical. Aqueous suppressants, bandage contact lenses, or pressure patches are often tried early on.…”

“…Until now, different methods have been described for adjustment of scleral flap tension and filtration [11,[29][30][31][32][33]. In our technique our objective was to improve the efficacy of trabeculectomy with the advantage of a potential space created by deep sclerectomy and we controlled the filtration rate by suture tension adjustment under drainage with an anterior chamber maintainer.…”
