Endoscopic examination of the lungs during bronchoscopy produces a considerable amount of endobronchial video. A physician uses the video stream as a guide to navigate the airway tree for various purposes such as general airway examinations, collecting tissue samples, or administering disease treatment. Aside from its intraoperative utility, the recorded video provides high-resolution detail of the airway mucosal surfaces and a record of the endoscopic procedure. Unfortunately, due to a lack of robust automatic video-analysis methods to summarize this immense data source, it is essentially discarded after the procedure. To address this problem, we present a fully-automatic method for parsing endobronchial video for the purpose of summarization. Endoscopicshot segmentation is first performed to parse the video sequence into structurally similar groups according to a geometric model. Bronchoscope-motion analysis then identifies motion sequences performed during bronchoscopy and extracts relevant information. Finally, representative key frames are selected based on the derived motion information to present a drastically reduced summary of the processed video. The potential of our method is demonstrated on four endobronchial video sequences from both phantom and human data. Preliminary tests show that, on average, our method reduces the number of frames required to represent an input video sequence by approximately 96% and consistently selects salient key frames appropriately distributed throughout the video sequence, enabling quick and accurate post-operative review of the endoscopic examination.

Abstract. We demonstrate a fast two-color widefield fluorescence microendoscopy system capable of simultaneously detecting several disease targets in intact human ex vivo lung tissue. We characterize the system for light throughput from the excitation light emitting diodes, fluorescence collection efficiency, and chromatic focal shifts. We demonstrate the effectiveness of the instrument by imaging bacteria (Pseudomonas aeruginosa) in ex vivo human lung tissue. We describe a mechanism of bacterial detection through the fiber bundle that uses blinking effects of bacteria as they move in front of the fiber core providing detection of objects smaller than the fiber core and cladding (∼3 μm). This effectively increases the measured spatial resolution of 4 μm. We show simultaneous imaging of neutrophils, monocytes, and fungus (Aspergillus fumigatus) in ex vivo human lung tissue. The instrument has 10 nM and 50 nM sensitivity for fluorescein and Cy5 solutions, respectively. Lung tissue autofluorescence remains visible at up to 200 fps camera acquisition rate. The optical system lends itself to clinical translation due to high-fluorescence sensitivity, simplicity, and the ability to multiplex several pathological molecular imaging targets simultaneously.

Introduction: Lung cancer may manifest in various ways. It may have typical symptoms such as weight loss, cough, heamoptysis and can be confused with pneumonia or it may manifest as a part of paraneoplastic syndrome. Occasionally it may be detected incidentally during evaluation for other medical illness. The early detection rate of lung cancer is rising due to cancer screening programmes, improvement in resolution of Computerised tomography scanners and also refinements in bronchosocpy techniques such as endobrochial ultrasound and electromagnetic navigation, fluoroscopy guidance and virtual bronchoscopy. The aim of this study was to correlate bronchoscopic findings with clinicoradiological profile of patients with suspected lung cancer visiting our Tertiary Health Centre, Nashik. Materials and Methods: This was a prospective study, conducted in a tertiary care medical college hospital over a period of 2 years. Patients were recruited from department of pulmonary medicine and referred cases from other departments were also included. Results: Complete data records from 52 patients were available for statistical analysis. Maximum numbers of patients were in age group 41-60. All bronchoscopies were done under local anesthesia and short conscious sedation. Majority of bronchoscopies were done in males. There were no fatal complications. Biopsy of visible growth was the most useful diagnostic tool followed by bronchoalveolar fluid. Squamous cell carcinoma was the most frequent diagnosis. Conclusion: Bronchoscopy is an excellent tool to evaluate lung carcinoma and multimodality approach is always helpful. Experience of bronchoscopist is crucial and accuracy of procedure increase with experience. Incidence of adenocarcinoma is increasing in India but squamous cell carcinoma remains most frequent lung cancer. Transbronchial lung biopsy, Transbronchial needle aspiration and brush biopsy, brochoalveolar fluid aspiration are helpful tools. However the yield can be enhanced by using Endo bronchial ultrasound (ebus), fluoroscopic guidance and electromagnetic navigation, virtual brochoscopy. There is good correlation between clinicoradiological and bronchoscpoic diagnosis and early diagnosis of lung malignancy will help improve survival and quality of life of patients..
