Florid, Papillary Endosalpingiosis of the Axillary Lymph Nodes

Abstract: A 55-year-old woman underwent radical mastectomy and axillary node dissection because of an invasive ductal carcinoma with neuroendocrine features. Histologically, all 22 sampled lymph nodes had widespread cystic inclusions lined by a regular, serous-type epithelium positive for cytokeratin-7, WT-1, CA125, and estrogen receptors. Papillary projections were found in the lumen of some cysts. The lesions were consistent with florid, papillary endosalpingiosis (FPE), a hitherto unreported condition in a supradiaph… Show more

“…This has been aptly called the secondary Müllerian system (Lauchlan et al, 1994), which is not restricted to the peritoneum, but also present in the urinary bladder (Donne et al, 1998), ureter (Nogales et al, 1999), pleura and even in the abdominal and axillary lymph nodes (Stolnicu et al, 2011).…”

“…Immunohistochemistry would not be contributive in this case. Florid papillary endosalpingiosis of the axillary nodes has also been reported before [16]. Morphology—recognizing the absence of overt atypia—is crucial in establishing the correct diagnosis.…”

“…Psammoma bodies, as seen in our case, have also been reported. 2,10 For benign lesions, the lymph node inclusion should morphologically be distinct from the primary lesion, may possess a myoepithelial layer, and lack marked cellular and architectural atypia, necrosis, and desmoplasia 1 ( Table 2). At times the identification of a basement membrane 4 may help to confirm the benign nature of the lesion.…”

“…At times the complete resemblance to that of the fallopian tube is not complete, with lack of intercolated cells and cilia, therefore the term "serous change," has also been reported. 2 Endosalpingiosis is commonly encountered in pelvic retroperitoneal lymph nodes, with an incidence of 11.4% in woman undergoing lymph node evaluation in association with radical hysterectomy performed for cervical carcinoma. 12 The finding of endosalpingiosis involving lymph nodes occurring outside of these regions and above the diaphragm is an extremely rare event.…”
