The high sensitivity of nucleic acid amplification tests such as ligase chain reaction (LCR) has the potential to simplify specimen collection for the microbiologic diagnosis of gonorrhea. We screened first-void urine specimens from 283 women attending a Birmingham, Ala., sexually transmitted disease (STD) clinic by using LCR and compared the results to those of cervical and urethral cultures for gonorrhea diagnosis. Fifty-three (18.7%) women had positive cervical cultures for gonorrhea, and 41 of the 53 (77%) also had positive urethral cultures. One additional patient had only a positive urethral culture (the cervical gonorrhea culture was negative). LCR testing of urine specimens for gonorrhea yielded positive results for 51 of 54 (94.4%) women with positive cervical or urethral cultures. Of 229 women with both urethral and cervical cultures negative for gonorrhea, 2 (0.8%) had positive urine LCR results as well. To resolve the discrepancies between urine LCR and culture results, LCR tests of simultaneously collected urethral and cervical swab specimens and LCR tests of the same urine specimens using different nucleotide primers were conducted. After evaluation of five discrepant results, the sensitivity, specificity, positive predictive value, and negative predictive value of LCR for the detection of gonorrhea in urine specimens were 94.6%, 100%, 100%, and 98.7%, respectively. We conclude that urine LCR testing for Neisseria gonorrhoeae is a practical alternative to culture for the detection of gonorrhea in women. Urine testing for STD diagnosis has the potential to simplify and expand the opportunities for STD screening and surveillance of women.

Comparison of GC-Lect and modified Thayer-Martin media for isolation of Neisseria gonorrhoeae

A new selective medium, GC-Lect, was compared with modified Thayer-Martin medium (MTM) for isolation of Neisseria gonorrhoeae. Cultures from 620 sexually transmitted disease clinic patients were directly inoculated onto both media, placed in candle extinction jars, and incubated. N. gonorrhoeae was isolated from 175 (29%) of 607 genital cultures, 3 (3%) of 88 pharyngeal cultures, and 6 (29%) of 21 rectal cultures. Ten cultures were positive only on GC-Lect, and 3 were positive only on MTM. In 3 of the 10 cultures positive only on GC-Lect, overgrowth of a Capnocytophaga sp. may have obscured growth on MTM. In this study, vancomycin-susceptible (MIC, <4.0 ,ug/ml) N. gonorrhoeae was isolated on both media, and none of the isolates missed by either medium were susceptible to vancomycin. No differences were noted between the two media in time required for isolation of N. gonorrhoeae. While isolation rates of N. gonorrhoeae were similar, suppression of nongonococcal bacterial species by GC-Lect was superior to that by MTM. GC-Lect is equal to MTM for detection of N. gonorrhoeae and is superior for suppression of normal flora.

HISTORICAL PERSPECTIVES ON THE TAXONOMY OF THE FAMILY NEISSERIACEAE Taxonomy of the Family Neisseriaceae The genus Neisseria belongs to the family Neisseriaceae (95), which has undergone many taxonomic changes (14, 15, 19, 74, 75, 80) that are summarized in Table 1. The genus Neisseria was assigned to the family Coccaceae until 1948 (74), when it was reassigned as the type genus in the family Neisseriaceae (75). The family Neisseriaceae at that time also contained the strictly anaerobic Veillonella spp. (14, 19). The family Neisseriaceae now contains the genera Neisseria, Moraxella, Acinetobacter, and Kingella (14), which are differentiated from each other by cell morphology, oxidase and catalase reactions, the presence of carbonic anhydrase, the production of acid from glucose, the ability to reduce nitrite, the presence of thymidine phosphorylase, nucleoside deoxyribosyl transferase, and thymidine kinase, and the presence of true waxes in the cell wall (14). The genus Neisseria contains species that are isolated from humans and other animals. The human species have undergone few taxonomic changes. The most notable change in the taxonomy of the family has been a result of genetic studies. These led to the reassignment of N. catarrhalis to the genus Branhamella (25) and the inclusion of B. catarrhalis as a subgenus in the genus Moraxella (14, 15). Because subgenus and subspecific epithets are not used (90), strains of B. catarrhalis should correctly be called Moraxella catarrhalis. However, because B. catarrhalis is distinctly different from the Moraxella spp. in cell morphology and has recently been recognized as a pathogen (21, 27, 68), the name B. catarrhalis is commonly used, although no formal request has been made to have the name conserved taxonomically.
