Current research indicates that there is a strong relationship between pediatric anxiety disorders and depression. Assessment measures show high rates of correlation between depression and anxiety and much of the overlap may be related to a common domain of negative affectivity. Anxious youth and depressed youth share a cognitive style marked by a negative bias in information processing. Anxiety disorders and depression are frequently comorbid in children and adolescents. About 25-50% of depressed youth have comorbid anxiety disorders and about 10-15% of anxious youth have depression. Twin and family studies have demonstrated that pediatric anxiety disorders and depression likely share some common genetic factors or influences. Selective serotonin reuptake inhibitors and cognitive-behavioral therapy have been shown in randomized controlled trials to be efficacious for both pediatric depression and anxiety disorders. Integrating the treatment literature with studies of phenomenology, biology and genetics indicates that pediatric anxiety disorders and depression may share a genetically determined neurobiological component that could involve neural circuits that include or are modulated by serotonergic neurons. This component could contribute to the negative affective temperament that appears to be common in both pediatric depression and anxiety disorders.

There are limited data on the neurocognitive correlates of childhood anxiety disorders. The objective of this study was to examine whether visual and verbal memory deficits of nonemotional stimuli are (1) a shared feature of three common childhood anxiety disorders (social phobia, separation anxiety disorder, and generalized anxiety disorder) or whether these deficits are restricted to specific anxiety disorders, and (2) present in offspring who possess at least one of the following established risk factors for anxiety disorders, parental history of panic disorder (PD), or major depressive disorder (MDD). One hundred and sixty offspring, ages 9-20 years, were recruited from parents with lifetime diagnoses of PD, MDD, PD plus MDD, or neither illness. Different clinicians blindly administered semistructured diagnostic interviews to offspring and parents. Verbal and visual memory subtests of the Wide Range Assessment of Memory and Learning were administered to offspring. The results showed that offspring with ongoing social phobia demonstrated reduced visual but not verbal memory scores compared to those without social phobia when controlling for offspring IQ, separation anxiety disorder, and generalized anxiety disorder. No other offspring anxiety disorder predicted memory performance. Neither parental PD nor parental MDD was associated with offspring memory performance. These findings are relevant to understanding the phenomenology of childhood anxiety disorders and may provide insights into the neural circuits underlying these disorders.

ResumoContexto: Transtorno de ansiedade social é um transtorno incapacitante e altamente prevalente em crianças e adolescentes ao longo da vida, de acordo com os critérios do DSM-IV, variando de 0,7% a 3,5%. Se não tratado, pode interferir no funcionamento emocional, social e escolar. Objetivos: Avaliar a evidência atual para a eficácia e efetividade de intervenções farmacológicas e psicoterápicas no tratamento do transtorno de ansiedade social na infância e na adolescência. Métodos: Pesquisas foram realizadas por meio do PubMed, Lilacs, PsycINFO e Cochrane Library até agosto de 2006 usando os termos "social anxiety disorder" OR "social phobia" AND "child*" AND "adolesc*". Resultados: Várias modalidades de tratamento, incluindo tratamentos psicoterápicos e farmacológicos, têm sido propostas para o tratamento desse transtorno. Terapia cognitivo-comportamental e farmacoterapia, principalmente com inibidores seletivos da recaptação de serotonina, são o tratamento de escolha para o transtorno de ansiedade social nessa idade.
