Focused Ultrasound and Lithotripsy

Abstract: Shock wave lithotripsy has generally been a first choice for kidney stone removal. The shock wave lithotripter uses an order of microsecond pulse durations and up to a 100 MPa pressure spike triggered at approximately 0.5-2 Hz to fragment kidney stones through mechanical mechanisms. One important mechanism is cavitation. We proposed an alternative type of lithotripsy method that maximizes cavitation activity to disintegrate kidney stones using high-intensity focused ultrasound (HIFU). Here we outline the metho… Show more

“…At higher intensities, ultrasound can generate temperatures sufficiently high to denature proteins and coagulate tissues. High-intensity ultrasound has been exploited for clinical applications to ablate kidney stones, fibroids, cancer, or cranial tumors and even to create irreversible functional brain lesions for movement disorders [18,[26][27][28]. At low intensities, tissue temperature changes are minimal and within the normal physiologic range [29][30][31][32].…”

“…Ikeda et al proposed high-intensity focused ultrasound (HIFU) for lithotripsy to maximize the cavitation effect using two acoustic waves with different frequencies: one to nucleate multiple bubbles, called bubble clouds, and the other one to excite the bubble dynamics [8]. HIFU also showed great potential as a noninvasive treatment as SWL with the accurate control of cavitation behavior [11,[102][103][104]. It is worth noting that excessive energy generated by SWL may result in considerable damage to organs, e.g., rupture of injury-prone blood vessels [105].…”

“…For example, high-intensityfocused ultrasound (HIFU) has been used clinically to ablate kidney stones, fibroids, and cranial tumors. [16][17][18] HIFU generates temperatures that cause damage to cells and proteins. Alternatively, low-intensity-focused ultrasound (LIFU) is considered harmless and safe for cells and has been used to disrupt the blood brain barrier by using microbubble interactions.…”
