This study sought to understand and describe the pregnancy and childbirth experiences of women with epilepsy (WWE). Methods: Data were collected from 2016e2017 through in-depth individual interviews with 12 WWE who experienced childbirth within 36 months. Verbatim transcripts were analyzed following Colaizzi's phenomenological analysis to uncover the meaning of the experiences of the participants. Results: The pregnancy and childbirth experiences of WWE were clustered into four theme clusters and 8 themes from 20 meaning units: 1) Feeling anxious due to unplanned pregnancy and unexpected changes; 2) Standing at crossroads that never guarantee satisfaction; 3) Carrying a burden of fearful expectation and daily routines; 4) Enjoying rewards of pregnancy and childbirth as a woman with epilepsy. Conclusion: WWE had strong anxiety about the possible abnormality of their babies during pregnancy. They had mixed feelings about delivery and had to make a tough decision about breastfeeding because of antiepileptic drugs use. After childbirth, they had increased fear about the possible inheritance of the illness and had a hard time managing the burden of childcare and seizure control. However, pregnancy allows these women to gain disease awareness and further appreciate the importance of their health. The study results indicate the need for multidisciplinary intervention for WWE, before, during, and after pregnancy to increase communication with health professionals. Especially, preconception counselling and education led by nurses are required.

Hormonal contraceptives are highly prevalent. Currently, little is known about Irish hormonal contraceptive trends to date since the 1995 British media contraceptive controversy. The aim of this study was to examine recent trends in contraceptive use in Ireland and to determine the frequency of co‐prescriptions with important interacting medications. Approximately 40% of the Irish population are prescribed 70% of total medicines under the Irish GMS scheme. Medicines were identified using the WHO Anatomical Therapeutic Chemical (ATC) classification system. Regression analysis was used to examine trends over time. Of all contraceptives dispensed in 2013, oral contraceptives were used the most (74%) and long acting reversible contraceptives (LARCs) the least (7.5%). Fourth generation combined oral contraceptives (COCs) predominated, although a slight significant decline was shown (P < 0.0001). Second and third generation COCs were significantly increasing and decreasing, respectively (P < 0.0001). Progestin‐only pills were significantly increasing (P < 0.0001 across age groups). Low rates of contraceptive co‐prescribing with important interacting drugs are shown. However, 93.6% of those on enzyme‐inducing anti‐epileptic medications were co‐prescribed ineffective contraception containing <50 μg oestrogen.Irish prescribing trends of second and third generation COCs have remained consistent since 1995. The slow decline in fourth generation COC uptake follows new evidence of an increased risk of venous thromboembolism (VTE) reported in 2011. The low, but increasing, uptake of LARCs is consistent with other countries. Co‐prescribing practices involving hormonal contraceptives requires continued vigilance. This study emphasizes the need to optimize co‐prescribing practices involving hormonal contraceptives and anti‐epileptic medications and highlights the need to address the barriers to the currently low uptake of LARC methods in Ireland.

A novel folic acid-imprinted quartz crystal microbalance (QCM) nanosensor in order to detect folic acid depending on the fabrication of folic acid-imprinted polymer film on a QCM electrode was developed with methacrylamidoantipyrine-gadolinium (III) [MAAP-Gd(III)] used as a monomer for metal coordination-chelation interaction to obtain a more selective molecularly imprinted polymers (MIPs). The developed folic acid-imprinted nanosensor on the MIP/QCM detection system showed outstanding properties such as a stronger affinity of 3.07 × 10 7 M −1 , a short response time of 10 min, wider linear range of 0-100 µM, a selectivity of k = 8.25 and lower detection limit of 0.0080 µM. After characterizing its all features, the newly designed folic acid-imprinted QCM nanosensor was utilized to detect folic acid level in commercial follow-on baby milk in order to determine its use in real samples.
