We report computed tomography (CT) findings for a rare case of follicular dendritic cell sarcoma of the greater omentum from a 47-year-old female patient. The tumor presented ash a palpable mass lesion in the umbilical region for the last two months. Multidetector CT scan of the abdomen showed a 14-cm soft-tissue mass with calcification and necrosis within the greater omentum. As a result, a follicular dendritic cell sarcoma should be considered in the differential diagnosis of a solitary omentum mass, especially one with coarse and chunk-like calcifications.

Extranodal follicular dendritic cell sarcoma (FDCS) is an extremely uncommon tumor with only a single case arising in the breast having been reported. We describe the clinico-pathological features of an additional FDCS of the lower outer quadrant of the right breast in a 40-year-old woman. The tumor showed three patterns of growth, i.e., diffuse, myxoid and fascicular. The neoplastic cells were large, polygonal, with a slightly eosinophilic cytoplasm and oval or convoluted nuclei. They were intermingled with small lymphocytes, plasma cells and a few bizarre multinucleated giant cells. In the fascicular areas, the cells were spindled, while in the myxoid areas they showed a dendritic-like appearance, with long cytoplasmic processes. Mitoses were numerous and often atypical. The neoplastic cells were intensely immunoreactive for CD21, S-100 protein and epithelial membrane antigen, and focally for CD35, CD68 and cytokeratins. Polymerase chain reaction analysis did not reveal any Epstein Barr virus genome in the neoplastic tissue. Electron microscopy highlighted numerous interdigitating cytoplasmic processes with intercellular junctions of the serrated, immature desmosomal or undifferentiated types. The post-surgical course of the patient was uneventful and she is currently free of disease 19 months after surgery.

FDCS mostly involves lymph nodes and extranodal sites, such as the oral cavity, tonsils, pharynx, and mediastinum as well as intra-abdominal organs [1,2], representing the omentum, as our peculiar case does, an exceptionally rare primary site [6,7]. Moreover, by pathological point of view, the present case shows aggressive histological features such as very atypical large plump ovoid cells along with bizarre pleomorphic nuclei, frequent atypical mitoses, and rare foci of tumor necrosis, which represent other atypical findings quite rarely observed in FDCS.…”

Follicular dendritic cell sarcoma (FDCS) is an uncommon tumor arising from follicular dendritic cells (FDCs) in the lymph nodes or in extranodal sites. We herein report an unusual case of a 30-year-old man who presented with a FDCS of the omentum and massive liver involvement. The tumor proliferation was a diffuse growth composed by large and highly pleomorphic cells. On immunostaining, B and T cell markers; CD30, myeloid, and histiocytic markers; and S100 were negative whereas CD21, CD23, CD35, clusterin, and fascin were positive. Unexpectedly, large spectrum cytokeratin clone AE1/AE3 was focally and variably expressed. Despite the latter atypical finding and the misleading morphology, immunohistochemistry allowed for a conclusive diagnosis of FDCS.
