Follicular lymphoma transforming to DLBCL and reverting back to follicular lymphoma at relapse—a case report

Abstract: Background Transformation of low-grade follicular lymphoma to high-grade diffuse large B cell lymphoma (DLBCL) is known. However, the opposite is not commonly reported. In this report, we present a case of follicular lymphoma that underwent transformation to DLBCL. Three years after treatment for histologic transformation, the patient presented again with low-grade follicular lymphoma at the same site which is unusual in the natural history of follicular lymphoma. Case presentation A 50-year-old female patien… Show more

“…[17][18][19] Risk factors for HT include a higher FLIPI score at diagnosis (Age > 60 years, >4 nodal sites, LDH elevated, Hemoglobin < 12 g/dL) and grade 3-4 FL. 5 There was a report about HT of FL to DLBCL with a simultaneous MYC and BCL2 translocation (double-hit). It is also important to keep in mind that HT to DLBCL has been shown to be associated with a poor prognosis, with a median survival time of 14−27 months.…”

“…3 Histologic transformation (HT) of FL, that is, evolution of low-grade FL to highgrade lymphoma, is considered to be one of the mechanisms contributing to the development of DLBCL during the clinical course. 4,5 Clinicopathological studies on ocular adnexal DLBCL (OA-DLBCL), however, are limited because they are less frequent than EMZL. 6 Flow cytometry (FCM) is useful for quantitative assessment of cell-surface markers in various leukocyte populations.…”
