Food allergy—From food avoidance to active treatment

Abstract: The prevalence of food allergy (FA) has increased too rapidly, possibly due to environmental factors. The guidelines recommend strict allergen avoidance, but FA is still the main cause of anaphylaxis in all age groups. Immunotherapy is the only treatment able to change the course of allergic disease, and oral immunotherapy (OIT) is the more effective route in FA. However, it carries the risk of adverse reactions, including anaphylaxis. To improve OIT safety, adjuvant therapy with the immunoglobulin E (IgE) mon… Show more

“…Previous research indicated positive effects of combining peanut OIT with an anti‐IgE treatment, such as omalizumab 8‐10 . Omalizumab lowers the sensitivity of mast cells and basophils, by capturing circulatory free IgE as well as downregulating the expression of the FcεRI on the basophils and mast cells and subsequently prevents the binding of IgE to the high‐affinity FcεRI on mast cells and basophils 11 .…”

“…Therefore, the transfer from food avoidance to active treatment is essential. Omalizumab and oral immunotherapy have been used for treating IgE-mediated food allergy, which has shown the potential to be used for CMPA treatment (Inuo et al, 2018;Costa et al, 2020). Nowadays, a few other active treatment strategies, such as probiotics and prebiotics (Qamer et al, 2019), transplantation of fecal microbiota (Feehley et al, 2019), and precise personalized designed diets, have been developed for the treatment of infants with CMPA (Figure 2).…”

“…It is understood that the key management for the long-term treatment of patients with a food allergy is food avoidance [122]. The biggest concern of this approach is the high risk of malnutrition and further exacerbating feeding difficulties among children [123], especially in picky eaters.…”
