Four decades of experience with carcinoid heart disease: An analysis of 84 patients

Abstract: Carcinoid heart disease (CHD) is a serious cardiac condition which is caused by elevated serotonin in the systemic circulation, secreted by neuroendocrine tumours (NET). It mostly affects the right-sided heart valves, where it causes fibrotic disturbances and is associated with worse survival. In this study, we describe a large cohort of patients with CHD and provide an insight into their survival over the past decades. All consecutive patients with a serotonin producing NET and CHD referred to the Netherlands … Show more

“…24 NT-proBNP levels have also been found to be associated with CHD progression and mortality. 19 Hence, NT-proBNP is also recommended as a routine biomarker for diagnostic and prognostic evaluation of CHD in patients with NETs (Table 1). 4,[24][25][26]…”

“…17,19,48,55 Subsequently, the proportion of patients with CHD recurrence on bioprostheses requiring reintervention has gradually increased, accounting for 5%-10% of all patients in recent studies. 19,48,67 The main pejorative prognostic factors of patients with CHDin addition to surgical replacementare older age, advanced New York Heart Association (NYHA) class, high NT-proBNP and 5-HIAA levels and right ventricular dilatation (Table 4). These features underline the role of clinical, biological and echocardiographic follow-up in these patients.…”
