Fractional Flow Reserve and Coronary Bifurcation Anatomy

Abstract: The new bifurcation model provides a comprehensive assessment of bifurcation anatomy. Compared with straight analysis, identification of lesions with preserved FFR values in obstructive bifurcation stenoses was improved. Nevertheless, accuracy was limited by using solely anatomical parameters.

“…Bifurcation lesions with side-branches ≥2 mm, as addressed in our present study, were also examined in the Z-SEAside and the SEAside studies [9, 25]. Recently, advanced, user-friendly three-dimensional reconstruction and analysis software for bifurcation lesions has become available [41], but such software was not available at the time of angiographic analysis of the TWENTE trial.…”

“…Then, using the local site network for QFR and AMR calculation, the data were transferred to the AngioPlus system (Pulse Medical Imaging Technology, Shanghai, China). Patients were excluded if QFR or AMR could not be calculated because of 1 the poor quality of angiographic images and 2 the presence of severe vascular curvature and overlap 7,13 …”

“…Subsequently, the analysis frame should have good contrast fill‐in and complete exposure of the lumen contour, with automatic delineation of the lumen boundaries of both the studied vessel and major side branches. Based on the Murray bifurcation fractal law, the reference vessel diameter was reconstructed, considering the step‐down phenomenon across bifurcations 13 . Finally, based on the fluid dynamic equations with the abovementioned hyperemic flow as the boundary condition, the pressure drop was calculated 14 .…”

“…In contrast, 3D-QCA was proposed to overcome some inherited limitations in 2D-QCA, such as out-of-plane magnification and foreshortening, which has a better ability to evaluate the geometric characteristics of the lesion 35. Tu et al36 also used 3D-QCA to evaluate the stenosis severity of bifurcation lesions (Fig. 2).…”
