Frailty Assessment Predicts Acute Outcomes in Patients Undergoing Screening Colonoscopy

Abstract: Compared to age and CCI, frailty status better predicts colonoscopy outcomes in older adults. Among adults undergoing colonoscopy, routine frailty screening should be considered for risk stratification. Additional prospective studies evaluating frailty measurements in endoscopy will further clarify its role in forecasting adverse events.

“…This benefit is opposed by 0.1 to 2 additional complications over the lifetimes of these 1000 individuals 45 . It was, however also shown, that risk of bleeding and perforation increased with age with the youngest population being at the lowest risk for complications 46 , 47 . These data, in our opinion, clearly depict that the favorable outcomes outweigh the harms of the procedure in young individuals and also led the USPSTF to change their recommendations in 2021 8 .…”

“…Gait speed is a critically important performance-based measure of the frailty phenotype, 2 based on independent associations with chronic disability, nursing home institutionalization, injurious falls, and other health outcomes. 3 Robust epidemiological studies 4 support the use of a combined age, sex, and gait speed algorithm to predict life expectancy, a variable useful for guiding medical decisions such as surgery, 5 medical testing, 6 and prescribing. 7 Barry and colleagues provide initial evidence supporting feasibility, validity, and acceptability of their radio-frequency identification (RFID)-based system by patients and providers in a real-world clinic.…”

“…4 Frailty is a state of decreased physiologic capacity and reserve in which patients are more vulnerable to acute stressors such as surgery. This is often associated with sarcopenia.…”

“…This issue of Digestive Diseases and Sciences features a study by Taleban and colleagues which addresses the problem of risk stratification by using frailty as a predictor of complications associated with colonoscopy. 4 Frailty is a state of decreased physiologic capacity and reserve in which patients are more vulnerable to acute stressors such as surgery. This is often associated with sarcopenia.…”
