Frameless Stereotactic Radiosurgery of a Solitary Liver Metastasis Using Active Breathing Control and Stereotactic Ultrasound

Abstract: Frameless stereotactic radiotherapy with the combination of ABC and BAT allows the delivery of high single doses to targets accessible to ultrasound with high precision comparable to a frame-based approach.

“…Sometimes, the IGRT system requires implanted fiducials in the tumor [40], and lipiodol is a direct surrogate for CBCT image guidance for radiotherapy of HCC after TACE [43]. Image-guided system associated with ABC can increase the overall precision of target volume [43,44,45]. Image-guided RA in combination with ABC may become an effective and accurate way to administer HCC radiotherapy.…”

“…In extracranial treatment this accuracy may be beneficial in SBRT [2,13] and intensity-modulated radiotherapy (IMRT) [6] with steep dose gradients between target volume and adjacent organs at risk. Dose-escalated IMRT treatment of paraspinal tumors or reirradiation of spinal metastases has been described with daily imaging and online correction of patient positioning errors [9,24,29].…”

“…While radiation therapy plays a major role in the treatment of malignant tumors, the irradiation of lung metastases was not considered to be an adequate measure until recently. With the advent of stereotactic treatment techniques the precision of beam application could be raised significantly [1], leading to the possibility of high-dose treatments even in radiosensitive organs like the lung. Its local effectiveness in lung metastases was shown, but mostly using hypofractionated treatment approaches, where the total dose was applied in three or more fractions [3,13,15,18].…”
