Giant cell tumor of the bones occurring in the first metacarpals frequently requires entire metacarpal resection due to the aggressive nature and high rate of recurrence. Bone reconstruction can be performed with autogenous bone grafts. Here we describe a new technique of reconstruction using a patient-matched three-dimensional printed titanium first metacarpal prosthesis. This prosthesis has a special design for ligament reconstruction in the proximal and distal portions. Good hand function and aesthetic appearance were maintained at a 24-month follow-up visit. This reconstructive technique can avoid donor-site complications and spare the autogenous bone grafts for revision options.

Background: Bone giant cell tumors are rarely found on the hand. Several treatments have been described but only the resection/ amputation of the affected segment can offer an effective cure in advanced cases. Various forms of reconstruction have been described, namely using a free graft of fibula. Case Report: The authors present a case of a man with an osteolytic and expansive lesion of the distal end of the second metacarpal that was treated with resection and autologous free fibular graft with good clinical and radiologic outcome. Conclusion: Free fibular graft is a possible solution to avoid ray amputation in cases of bone tumors in the metacarpals.

Ewing sarcoma is a primary bone malignancy that rarely occurs in the hand. Resection and reconstruction will usually result in reduced or loss of thumb function. We describe a case of successful transplantation of nonvascularized osteoarticular second metatarsal autograft following wide resection of Ewing sarcoma of first metacarpal in an 11-year-old girl. The capsule of the graft’s metatarso-phalangeal joint was repaired to achieve mobility of the joint. This enabled preservation of thumb function by a relatively simple surgical technique. Detailed surgical procedures and excellence function after 2 years following surgery are described.
