Frequency and Management of Sleep Disturbance in Adults with Atopic Dermatitis: A Systematic Review

Abstract: IntroductionIntense nocturnal pruritus as well as the complex pathophysiology of atopic dermatitis (AD) can severely affect sleep and become a major factor in negatively impacting quality of life in adults. However, much of the literature on sleep disturbance in AD patients is on the pediatric population, and it is not well studied in adults. Furthermore, limited studies are available to guide effective management of sleep disturbance in AD in general. We review the literature to present the studies that have … Show more

“…Patients with moderate‐to‐severe atopic dermatitis who need systemic therapy experience a significant burden of disease . Atopic dermatitis symptoms can profoundly affect sleep, daily functioning, mental/emotional state, concomitant medication use and HRQoL, particularly in patients with untreated or inadequately treated atopic dermatitis .…”

“…Patients with moderate‐to‐severe atopic dermatitis who need systemic therapy experience a significant burden of disease . Atopic dermatitis symptoms can profoundly affect sleep, daily functioning, mental/emotional state, concomitant medication use and HRQoL, particularly in patients with untreated or inadequately treated atopic dermatitis . Improvement in patient‐reported outcomes was significant in both the dupilumab + TCS treatment groups, except for the proportion of patients who achieved HADS‐A and HADS‐D scores < 8 (among patients with HADS‐A or HADS‐D ≥ 8 at baseline), which was significantly greater with dupilumab q2w + TCS, but not qw + TCS; however, both dupilumab + TCS dose regimens showed significant improvement in mean change in HADS score from baseline.…”

“…Pruritus, scratching and disturbed sleep are known to have major impact on QoL in patients with AD . Hence, assessment of these symptoms is important when evaluating treatment response in the clinical setting.…”

“…Pruritus, scratching and disturbed sleep are known to have major impact on QoL in patients with AD. 16 Hence, assessment of these symptoms is important when evaluating treatment response in the clinical setting. The finding that the median pruritus score was reduced from 8 at baseline to 2 at 1-and 3month follow-up, and that median sleep score was reduced from 7 at baseline to 0 at 1-and 3-month follow-up, emphasizes how treatment with dupilumab markedly improves key symptoms of AD and positively influences patients' daily living.…”

“…Some features of the clinical presentation of atopic dermatitis contribute to this: patients with eczematous lesions on the hands and feet may have limited physical activity because of that; fatigue from sleep disturbances and depression may be indirectly associated with reduced physical activity; at the same time, heat, sweat and exercise are the most frequently reported circumstances leading to exacerbation of symptoms. Additionally, adult patients with atopic dermatitis, apart from reduced physical activity, also have significantly higher cardiovascular risk 53,54,55 .…”
