Frontal intradiploic meningioma with progressive intracranial invasion

Abstract: Rationale:Intradiploic meningiomas are a subset of extradural meningiomas that arise in the skull. They are mostly localized in the frontoparietal and orbital regions, and they are often mistaken for primary bone tumors.Patient concerns:The patient was a 48-year-old man with headache and a 12-year history of frontal cranium occupation, which was first discovered in 2005 and enlarged in 2009. The patient had a history of fracture in his frontal cranium 12 years ago. The computed tomography and magnetic resonanc… Show more

“…A summary of patients’ characteristics in selected papers are reported in [ Table 1 ]. [ 2 - 43 ] A total of 41 studies were selected, including 59 patients, 32 females (54.2%), 26 males (44%), and one patient of unknown sex, with a female/male ratio of 1.2/1. The mean age of the patients is of 47.69 years, ranging from 3 to 84 years.…”

“…It is like the primary intradural meningiomas except for the dural tail sign, which could not be seen in intradiploic meningiomas. [ 43 ] Furthermore, scintigraphy and positron emission tomography CT (PET-CT) can be used for the differential diagnosis, with bone scintigraphy that shows an increased metabolic activity of the specific bone region when positive. [ 3 ]…”

“…Primary interosseous meningiomas develop from intraosseous meningeal cells. It may be either multipotent mesenchymal cells that differentiate into meningocytes; or sequestration of meningocytes during suture fusion during intra-uterine development or during head injury with fracture [6][7][8].…”

“…Surgical resection should be as complete as possible, taking all the bone involved. A bone reconstruction could be considered immediately or in a second time [6,8,11]. An extemporaneous histology often helps to take the decision to make or not this plasty at the same time operative.…”

“…Symptoms could vary depending upon location and size of the tumor and involvement of the adjacent structures. Most IOMs exist as painless expanding masses without abnormal neurological manifestations (32)). In four patients, expansion, exophthalmos, and orbital swelling associated with soft tissue components were observed in the affected bone.…”
