Fulminant Mycosis Fungoides with Tissue Eosinophilia: A Unique Presentation of Two Cases with Acro-Periorbital Ulceration and An Aggressive Clinical Course

Abstract: We describe two unique cases of fulminant mycosis fungoides with remarkably similar and aggressive clinical courses resulting in death. Both cases demonstrated ulcerated palmar and periorbital plaques and marked tissue eosinophilia, which was confirmed by T-cell receptor γ chain gene rearrangement studies to display identical monoclonality at temporally and anatomically distinct sites. Dense eosinophilic infiltrates on biopsy led to misdiagnosis of inflammatory dermatoses in both instances. While mycosis fungoi… Show more

“…Several studies indicate the loss of CD7 gene expression in the abnormal T cells of patients with MF (Abel, Wood, Hoppe, & Warnke, 1985;Feng et al, 2010;Jamal et al, 2001;Kelemen, White, Gatter, Braziel, & Fan, 2010;Matsuzawa et al, 2015;Merrill et al, 2017;Oshtory, Apisarnthanarax, Gilliam, Cooper, & Meyerson, 2007;Pearson, Fujita, & High, 2017). The skin infiltration in MF contains a high number of CD7 − T cells with high expression of CLA.…”
