Functional Outcomes in Duchenne Muscular Dystrophy Scoliosis

Abstract: Surgery in patients who had Duchenne muscular dystrophy with scoliosis improved function and decreased the rate of deterioration of forced vital capacity compared with patients treated conservatively. However, the muscle power and forced vital capacity decreased in both groups.

“…28 Previously published guidelines address respiratory management of patients undergoing surgery, including indications for preoperative training in the use of assisted cough devices and non-invasive ventilators. 13 For patients who are cognitively impaired and unable to reliably perform pulmonary function testing, preoperative polysomnography might be helpful.…”

“…28,104,105 Posterior spinal instrumentation and fusion are recommended in non-ambulatory individuals who have a spinal curve in the sitting position greater than 20-30°, who have not yet reached puberty, and who have not been treated with corticosteroids because the curve is expected to progress. Although patients treated with corticosteroids can still develop scoliosis, the progression might be less predictable, so observation for clear evidence of progression is a reasonable approach before intervening.…”

“…The effect of neuromuscular scoliosis surgery on pulmonary function has only been studied in patients with progressive muscular dystrophies such as Duchenne and spinal muscular atrophy [714]. In adolescent idiopathic scoliosis (AIS), increased spinal deformity is associated with decreased pulmonary function and patients who have undergone posterior spinal fusion with pedicle screw instrumentation have improved pulmonary function after surgery [1519].…”
