Functional recovery of stored platelets after transfusion

Abstract: Our study shows recovery of PLT function after transfusion in patients with thrombocytopenia. The majority of functional PLTs measured after transfusion most likely represents stored transfused PLTs that regained functionality in vivo. The difference in baseline P-selectin expression in vivo versus ex vivo suggests a rapid clearance from circulation of PLTs with increased P-selectin expression.

“…The clot formation capacity of the samples showed minimal changes over storage time, which is in agreement with an earlier study analyzing TEG parameters of platelet concentrates in the absence of RBCs and plasma . We can only speculate that this is indicative of an actual platelet transfusion , showing that after transfusion, the platelet quality remains affected despite of the interaction with RBCs and plasma. More studies are needed in this respect.…”

“…Multiple in vitro and in vivo studies have indicated that RBCs and plasma affect platelet function and haemostasis . When stored platelets are ex vivo mixed with whole blood, the agonist‐induced CD62P expression and aggregation improve , which implies the influence of RBCs and/or plasma on platelet function. Even while the PAS‐C–platelets and plasma–platelets were mixed with RBCs and plasma, in our experiment, which may partially recover platelet function, we still observed noticeable differences.…”

“…The most commonly used in vivo measurements are platelet recovery and survival following transfusion, as mandated by the FDA (66.7% recovery of the same subjects fresh platelets after 24 hours and 58% recovery of the same subject's fresh platelets) [136]. However, platelet components in routine clinical use can fail to meet these criteria, highlighting a need for alternative criteria that provide information regarding platelet function following transfusion [137140]. Nonetheless, there are strong correlations between some in vitro measurements and platelet recovery and survival in vivo.…”

“…Even with these measures, the maximum shelf life of standard RTPs is between 3 and 5 days in most countries, but may be extended to 7 days with point‐of‐release bacterial testing. The function of RTPs also declines during storage because of metabolic changes that impair platelet activation responses, known as the “platelet storage lesion,” which is only partially reversible after transfusion …”
