Fungus colonisation of pulmonary rheumatoid nodule

Abstract: This report describes a 69 year old woman, suffering from active rheumatoid arthritis since the age of 60 and presenting with severe dyspnoea and cough. A computed tomography scan of the chest showed multiple bilateral pulmonary nodules, sometimes cavitated, associated with reticular opacities and pleural effusion. A videothoracoscopic excision of a cavitated nodule was performed. Seven days after surgery, a right pneumothorax developed, and the patient died of septicaemia one month later. Microscopically, the… Show more

“…Aspergillus nodules (figures 6 and 13) are very similar in appearance to malignancy, coccidioidal nodules [75], NTM [76] and actinomycosis [77], as well as rheumatoid nodules [78,79]. Most such lesions are rounded in appearance and may have low attenuation or cavitation within.…”

“…Although fungal colonization of rheumatoid nodules has been reported twice in the past, 1,2 this case appears morphologically distinct and, as a result, raises several further issues with regard to this rare entity. In one report, an eosinophilic infiltration around the edge of the colonized rheumatoid nodule, and in the airways, was described 1 but, to the best of our knowledge, an eosinophilic pneumonia adjacent to, and distinct from a rheumatoid nodule colonized with aspergillus has not been reported.…”

“…Sir: Fungal colonization of pulmonary rheumatoid nodules has been reported only twice in the past 1,2 . We report a case of aspergillus colonization of rheumatoid nodules occurring in the presence of a marked eosinophilic infiltrate within the nodules and an eosinophilic pneumonia both adjacent to, and distinct from, the rheumatoid nodules.…”
