This paper examines mental health nurses' experiences of physical restraint in an acute inpatient psychiatric setting using Van Manen's descriptive hermeneutic phenomenological methodology. The aim was to understand the nurses' experiences of physical restraint. One overarching theme emerged from the analysis: It's part of the job. This theme had a subtheme of Control which was constituted by the Conflicted Nurse and the Scared Nurse. The findings suggest that mental health nurses are very uncomfortable with physical restraint despite it being taken-for-granted as integral to their role. The nurses experienced conflict and fear associated with the procedure and would prefer to utilize other de-escalation skills if it was possible. The main source of conflict related to the imperative to maintain control and the professional values of the therapeutic relationship. While the nurses could see no viable alternative in some situations, the paper concludes that while environmental issues impact on the practice of physical restraint mental health nurses need to practice it with as much care and humanity as possible.

ÖzetGözlem, kimyasal ve fiziksel tespit, izolasyon gibi kontrol yöntemlerinin yerine kullanılan bir kontrol yöntemidir. Gözlem özellikle birçok ülkede psikiyatri kliniklerinde akt dönemdeki bakımda intihar ve saldırgan davranışları olan hastaların güvenliğini sağlamak için bir müdahale yöntemi olarak kullanılmaktadır. Özellikle psikiyatri kliniklerinde profesyonel bir kontrol yöntemi sağlayan hemşire-lik gözlemlerinin kullanılması önemli bir konudur. Bu makalenin amacı akut psikiyatrik bakımda bir kontrol yöntemi olarak kullanılan gözlem hakkında literatür bilgisini gözden geçirerek, konu ile ilgili çalışma sayısının kısıtlı olduğu ülkemizde konunun önemine dikkat çekmektedir. Makalede akut psikiyatrik bakımda risk değerlendirme, karar verme, gözlem düzeyleri, gözlem uygulaması, gözle-min etik yönü ve yapılan çalışmaların sonuçları ele alınmıştır.Anahtar sözcükler: Psikiyatri kliniği, kontrol yöntemi, gözlem, psikiyatri hemşireliği. AbstractObservation is a method that is used in place of other control methods such as chemical and physical detention, isolation. Observation is used especially as an interference method to ensure the safety of the patients with suicidal and aggressive behaviors in acute psychiatric care in many countries. Especially in acute psychiatric wards using observations of nursing as a professional control method is an important issue. This article aims to draw attention to the importance of the subject in our country about using nursing observations as a control method in acute psychiatric care from the view of the literature. In this article several studies related to risk assessment, decision making, the levels of observation, the application of observation and the ethical aspects of observation on acute psychiatric care have been discussed.

This paper reviews the literature pertaining to the practice and experience of restraint and seclusion in inpatient psychiatric settings. These are physical practices typically used by nursing staff in order to manage violence within such services.The available quantitative literature demonstrates the variability in the prevalence of the practices and attitudes towards them. Qualitative research has focussed on the experience of the practices and the different but largely negative impacts they have. Many of these qualitative studies produce interesting and valuable findings; however the literature does not account for the role that language may play in constructing the experience and meaning of the practices for those involved. This and other limitations in the research are described and direction for further research indicated.
