Gallbladder volvulus (GV) or gallbladder twist, is a rare surgical emergency. The demographic tendency is septua-and octogenarians of the female sex and its overall incidence is increasing in laparoscopic era. GV is frequently undiagnosed before surgical procedure. However, clinical presentation and associated imagistic findings can lead to an accurate diagnosis, when there is a index of suspicion for gallbladder volvulus. When the diagnosis has been established preoperatively laparoscopic cholecystectomy can be performed safely. We discuss herein two cases of elderly, fragile women presenting to the emergency department complaining of sudden onset right upper quadrant abdominal pain with suspicion acute cholecystitis. The last case is diagnosed and treated by laparoscopic approach.
