Gamma Knife Radiosurgery in the Management of Arteriovenous Malformations of the Basal Ganglia Region of the Brain

Abstract: In our experience, the different clinical, epidemiological and anatomico-functional characteristics of BGAVMs and cortical AVMs do not seem to influence the radiosurgical outcome, since complete obliteration and permanent neurological sequelae rates were very similar in these two subgroups of patients. GK may be considered the first choice treatment modality for BGAVMs with < 10 cc volume, since it offers a very high cure rate, very low permanent morbidity, and zero mortality. Although haemorrhagic onset is mu… Show more

“…20,21,[24][25][26][27] Briefly, the MRI-compatible Leksell Model G stereotactic frame (Elekta Instruments) was applied to the patient's head. Then, neuroradiological localization was routinely performed using stereotactic 2D cerebral angiography or high-resolution magnification subtraction stereotactic angiograms.…”

“…8,25,41 Therefore, the "gold standard" treatment should be represented by complete surgical removal, with the aim of removing the risk of further, devastating hemorrhagic strokes. However, cAVMs are more frequently located in deep regions or in eloquent brain areas in children and adolescents (up to 70%-90% of the time) than in adults, 14,24,25,27,42,44 exposing these younger patients to high morbidity and mortality risks in the event of surgery.…”

“…1,18,19,25,33,35 Pollock et al 33 reported that total obliteration of AVMs in the basal ganglia (10 cases), thalamus (30 cases), and brainstem (16 cases) was achieved in 24 (43%) of 56 patients after primary SRS at a median of 45 months. The actuarial rate of an AVM cure (including those confirmed by both angiography and MR imaging) after one or more radiosurgical procedures was 47% and 66% at 3 and 4 years after radiosurgery, respectively.…”

“…1,18,19,25,33,35 The goal of SRS is complete obliteration of the AVM nidus while avoiding postprocedural ARE. Total obliteration rates after a single procedure vary between 60% and 80% over a 5-year period.…”
