Cytomegalovirus (CMV) infections are mostly seen in immunocompromised patients. However, unusual manifestations or complications of acquired CMV infections in immunocompetent patients are rarely reported. CMV-related hemorrhagic cystitis is extremely rare but should be considered even in immunocompetent patients. We present a case of a 3-year-old immunocompetent boy with intermittent, terminal gross hematuria lasting for 1 month. There was no history of genitourinary trauma or stone disease. Urine analysis revealed hematuria with eumorphic red blood cells and no proteinuria. Urine culture was negative. Ultrasonography showed increased bladder wall thickness and irregularity at inferior of bladder. Cystoscopy revealed hyperemia and edema. Histopathological examination was consistent with CMV infection, viral DNA by polymerase chain reaction in peripheral blood and urine were positive. Clinical, laboratory, and imaging features pointed towards hemorrhagic cystitis due to CMV. He was followed-up with no treatment. After 1 month, repeated investigations showed complete resolution of finding. This is a rare description of an immunocompetent child with CMV-induced cystitis.

IntroductionCytomegalovirus generally causes self-limited, mild and asymptomatic infections in immunocompetent patients. An aggressive course in immunocompetent healthy patients is unusual.Case presentationWe report the case of an immunocompetent 16-year-old Egyptian boy with encephalitis, acute renal failure, and acute hepatitis triggered by viral infection with a complete recovery following antiviral treatment.ConclusionWe believe that this case adds to the understanding of the molecular biology, clinical presentation and increasing index of suspicion of many viral infections.

ÖZSitomegalovirüs (CMV) enfeksiyonu tüm dünyada yaygın olarak görülmektedir. Sağlıklı çocuklarda primer enfeksiyon çoğunlukla semptom vermeden kendine sonlanan bir durum ortaya çıkardığından, enfeksiyonun immünkompetan çocuklardaki belirti ve tedavisi ile ilgili kısıtlı sayıda çalışma mevcuttur. Sağlıklı çocuklardaki CMV enfeksiyonlarında tedavide gansiklovir kullanımı halen tartışmalıdır ve yalnızca seçi-li olgularda uygulanır. Bu makalede öncesinde sağlıklı olan 52 günlük bir kız hastada gelişen ağır CMV enfeksiyonu olgusu sunulmuştur. Olgunun izleminde uzamış ateş ve derin trombositopenisi gelişmesi üzerine kısa süreli gansiklovir tedavisi başlanmış ve hızlı klinik yanıt alınmıştır. CMV enfeksiyonu immün yetmezliği olmayan çocuklarda çoğu zaman hafif geçmesine rağmen, olgular ağır klinik bulgular yönünden yakından izlenmeli ve gerekli durumlarda antiviral tedavi düşünülmelidir.Anahtar kelimeler: Sitomegalovirüs, sağlıklı çocuk, gansiklovir, trombositopeni ABSTRACT Cytomegalovirus (CMV) infection is a common disease worldwide. While infection in immunocompetent individuals has traditionally been considered a benign and selflimited condition, a few studies have reported the clinical manifestations and treatment of CMV infection among immunocompetent patients. Ganciclovir treatment in children with cytomegalovirus infection is still controversial and it is used only in selected cases. We report here a case of a previously healthy 52-day-old girl who developed severe CMV disease with prolonged fever and profound thrombocytopenia. Short-term intravenous ganciclovir treatment was associated with immediate and sustained resolution of the symptoms as well as a sharp decrease of CMV viremia. Antiviral therapy should be considered in select cases of severe primary CMV infection in immunocompetent children.
