Gastric amyloidosis presenting as acute upper gastrointestinal bleeding: a case report

Abstract: Background Amyloidosis is characterized by extracellular tissue deposition of fibrils, composed of insoluble low-molecular-weight protein subunits. The type, location, and extent of fibril deposition generates variable clinical manifestations. Gastrointestinal (GI) bleeding due to amyloid deposition is infrequent. Previous literature describes upper GI bleeding (UGIB) in patients with known amyloid disease. Here, we describe a case of recurrent UGIB that ultimately led to a diagnosis of GI amyl… Show more

“…Two other case reports described findings consistent with MGUS [14,15]. These patients presented at different stages of their disease, ranging from having an incidental finding [11,15] to presenting with severe complications such as hematemesis, anemia, and weight loss [10,[12][13][14]. Furthermore, four of these six patients improved with treatment [10,11,14,15] while another two succumbed to the disease [12,13].…”

“…Case reports concerning GI amyloidosis have been reported on multiple myeloma, MGUS, but have not been associated with plasma cell dyscrasia [10][11][12][13]. Two other case reports described findings consistent with MGUS [14,15].…”

“…Only nine cases were included in our review (reasons for exclusion were: baseline diagnosis of systemic amyloidosis in 29 cases, and in the other two cases, the presentation was not GI bleed). Of the nine cases, five were women and four were men, they all presented with gastrointestinal bleeding, with or without a previous diagnosis of multiple myeloma [11][12][13][14][15][16][17][18][19]. The GI bleed led to upper endoscopy and/or colonoscopy, where further GI pathology revealed amyloidosis.…”
