Gastric calcifying fibrous tumor: A case of suspected immunoglobulin G4-related gastric disease

Abstract: Gastrointestinal lesions resulting from immunoglobulin G4-related disease are classified into two types: One is a gastrointestinal lesion showing marked thickening of the wall, and the other is an IgG4-related pseudotumor. We report the case of a woman with gastric calcifying fibrous tumor undergoing endoscopic resection that contained 62 IgG4+ plasma cells per high-power field and an IgG4-to-IgG ratio of 41% in lesional plasma cells, which shared clinical and histopathological features associated with gastric… Show more

“…Kuo et al described splenic angiomatoid nodular transformation associated with disseminated abdominal, gastric, and small intestinal calcifying fibrous tumors, which all demonstrated IgG4-positive plasma cells [14]. Subsequently reported gastric calcifying fibrous pseudotumors have been reported to harbor IgG4-positive plasma cells in some but not all cases [10,15]. Our IHC studies did not demonstrate substantial IgG4+ cell infiltration.…”

“…In 2013, the first case of gastric CFT was treated with ESD [ 20 ]. Two patients were successfully treated with endoscopic full-thickness resection (EFR) in 2014 [ 22 ]. EFR may also be another choice for a patient with SMT arising from the muscle layer.…”

“…Some researchers have reported CFT was associated with immunoglobulin G4-related disease (IgG4-related disease) and suggest corticosteroid treatment, but radical tumor removal by surgery is the major treatment method, because postoperative histopathology and immunohistochemistry can obtain a definitive diagnosis. [16,17]…”
