Gastric cancer treatment in the world: Germany

Abstract: Epidemiologically, around 15,500 persons per year contract gastric cancer with continuously decreasing incidence and a 5-year survival rate of only 30% to 35%. Contrary to the Asian countries, there are no prevention programs for gastric cancer in Germany, which leads to the disease frequently being diagnosed in locally advanced stages and predominantly being treated with multimodal therapy concepts. Complete (R0) resection is the therapy of choice for resectable gastric cancer. Special forms of gastric cancer… Show more

“…It has the characteristics of easy invasion and metastasis, and a very low early diagnosis rate [ 2 ]. Despite the gradual decline in incidence and mortality of gastric cancer in recent years, the prognosis of patients with gastric cancer has not been significantly improved [ 3 ]. Thus, to explore biomarkers that can aid in the prognosis of gastric cancer not only would help clinicians predict prognosis and establish individualized treatment plans, but also would help clarify the mechanism of gastric carcinogenesis and development, which has important significance for formulating the treatment strategy of gastric cancer.…”

“…Die Etablierung von Screening-Programmen hat insbesondere in Korea und Japan dazu geführt, dass das Magenkarzinom oftmals noch im frühen Stadium entdeckt und behandelt werden kann [6,7]. Dahingegen befindet sich mehr als die Hälfte der in Deutschland erstdiagnostizierten Magenkarzinome bereits im lokal fortgeschrittenen Stadium (cT3 oder höher) [8].…”

“…Radical resection is widely considered as the most effective treatment for advanced gastric cancer [3,4]. In China, advanced gastric cancer with extensive invasion of gastric wall is more common as compared with other Asian countries, and most of these patients usually have to undergo total gastrectomy, regardless of open or laparoscopic approach, to achieve complete (R0) resection [5]. However, the reconstruction of digestive tract after total gastrectomy, which is almost exclusively represented by conventional Roux-en-Y esophagojejunostomy in China, is linked with various early and late postoperative complications and discomfort, such as dietary restriction, reflux esophagitis, dumping syndrome, anastomotic stenosis, and malnutrition [6].…”
