Gastric cryptosporidiosis: correlation between intensity of infection and histological alterations

Abstract: These findings suggest that, in individuals with AIDS, cryptosporidiosis and severe immunodepression, upper endoscopy with random gastric biopsies should be performed, even in the absence of endoscopically appreciable lesions. The diagnosis of gastric cryptosporidiosis has to rely on histological findings, and pathologists must be aware of the fact that Cryptosporidium parasites are mainly found in areas showing reactive hyperplasia.

“…Histological examination shows Cr yptosporidium parasites mainly on epithelial cells covering gastric pits and stained by H&E [37] . Rivasi et al [38] have demonstrated a close relationship between the intensity of Cryptosporidium parvum infection and the degree of histological alterations. They did not find, however, a clear correlation between the endoscopic and histological alteration types found [38] .…”

“…On the other hand, in vitro studies have indicated that cryptosporidium infection induces an apical defect in the host cell that causes epithelial cell death (13) . Histopathological studies of the gastric mucosa associated with the presence of C. parvum as the unique pathogen have shown nonspecific inflammation together with epithelial reactive changes (20,21) . RIVASI et al (20) demonstrated a close relationship between the intensity of C. parvum infection and the degree of histological alterations.…”

“…Gastric infection has preference for the antrum and pylorus, with rare examples affecting the corpus. 178,182 When organisms are found in the stomach, they are almost invariably present in concurrent duodenum and/or colon biopsies. 178,182 The opposite is not true because only a proportion of patients with duodenal cryptosporidiosis have gastric involvement.…”
