Gastroduodenal involvement in disseminated strongyloidiasis

Abstract: Here, we report the case of a 57‐year‐old Peruvian man on long‐term corticosteroid therapy for a drug allergy and proton pump inhibitors for chronic dyspepsia symptoms. Upper gastrointestinal endoscopy showed multiple white granular lesions and widespread erosions in the stomach. Our findings indicate that gastric strongyloidiasis should be carefully considered in high‐risk patients even if endoscopic findings are nonspecific.

“…The case we described had gastric strongyloidiasis as part of his disseminated strongyloidiasis. Cases of disseminated strongyloidiasis with gastroduodenal involvement, including multiple granular lesions, ulcerations, and even duodenal obstruction, have been reported in the literature [ 9 , 10 ]. However, this is the first case of disseminated strongyloidiasis in which a patient presented with an ulcerated gastric mass and iron deficiency anemia.…”

“…1 Patients may benefit from either serology or tissue biopsy during EGD for patients whom a physician has a high suspicion may be infected with Strongyloides . 6 Patients with eosinophilia either immigrating from or recently traveling from an endemic region should receive a stool microscopy test, serological test, or EGD with biopsy to look for Strongyloides infection before starting steroids because of the high risk of fatality due to hyperinfection with steroid use. Eosinophilia can be obscured by corticosteroid courses, as seen in this patient.…”
