Gastropleural fistula after single anastomosis gastric bypass. A case report and review of the literature

Abstract: HighlightsWe are reporting a very rare complication of bariatric surgeries. Most of reported cases of GPF were post sleeve gastrectomy. We could not find cases of GPF post minigastric bypass like our case. The presentation was very late in this case (2 years post surgery). The findings can be subtle and might be missed on some diagnostic tools.

“…It has been found to be associated with chest-abdominal trauma or complications following operations involving the lungs, diaphragm or enteric organs such as lung resection or diaphragmatic hernia repair 15–17. In recent years, GPF from bariatric surgery-related complications in morbid obesity cases have become the most common cause of GPF in adults,18–20 while the main cause of this fistula in children is still from infection causes 21 22…”

“…The first case of gastropleural fistula was described by Markowttz and Herter in 1960 [5]. To date, multiple etiologies of gastropleural fistulas have been described in the literature; one of which is bariatric surgery [6, 7]. Sleeve gastrectomy has been particularly identified to be one of the causes accounting for the development of gastropleural fistulas [8].…”

“…Late complications, as in the case presented, including gastro-bronchial or gastro-pleural fi stulas, are described after sleeve gastrectomy 2,3 . Oesophageal-pleural fi stula implies the existence of a pathological communication between oesophagus and pleural cavity, and it usually leads to life-threatening complications 9 . Th e exact mechanism for these fi stulae's appearance is not clear, but it is believed to be related with some postoperative leaks, which lead to local abscesses formation and creation of a pathological tract between oesophagus/stomach and pleural cavity or a bronchus 9 .…”
