Gender issues of antibody-mediated diseases in neurology: (NMOSD/autoimmune encephalitis/MG)

Abstract: Neuromyelitis optica spectrum disorder (NMOSD), autoimmune encephalitis (AE), myasthenia gravis (MG) and Lambert-Eaton myasthenic syndrome (LEMS) are antibody-mediated neurological diseases. They have mostly female predominance, affecting many women during childbearing age. Interactions between the underlying disease (or necessary treatment) and pregnancy can occur in every of these illnesses. Herein, we present the characteristics of NMOSD, AE, MG and LEMS in general, and review published data regarding the i… Show more

“…Autoimmune encephalitides are known to have varying gender frequencies based on their subtypes; however, most cases occur in women. Most notably the contactin-associated protein-like 2 (CASPR-2) mediated encephalitis is known to have a skewed female-to-male ratio of 1:9 and also often presents in older adults [ 8 ].…”

“…Leucine-rich glioma-inactivated protein 1 (LGI-1), contactin-associated protein-like 2 (CASPR-2), or seronegative subtype. Both LGI-1 and CASPR-2 are found within the VGKC protein complex [ 8 , 9 ]. In our case, both LGI-1 and CASPR-2 antibodies were found to be negative; however, VGKC protein complex antibody levels were elevated.…”

“…Myasthenia gravis (MG) is an autoimmune disease that is mediated by autoantibodies and involves neuromuscular junctions [ 1 ]. It has mostly female predominance [ 2 ], ocular muscle weakness is the most common first symptom, and gradually extends to limb, bulbar, and ventilatory muscles, resulting in generalized MG (GMG) [ 3 ]. A series of autoantibodies (Abs) related to MG have been found, such as acetylcholine receptor (AChR), muscle-specific kinase (MuSK), and low-density lipoprotein receptor-related protein 4 (LRP4) Abs [ 4 ].…”

“…106 Thirty-five to sixty percent of anti-NMDAR encephalitis patients have potential tumors, 96% of which are ovarian or extra-ovarian teratomas and tumors. 107,108 In addition to removing the tumor to eliminate the autoantibody production, TPE is usually the first-line treatment for anti-NMDAR encephalitis. Although some case reports have indicated that TPE treatment is ineffective for the disease, Cancer Management and Research 2022:14 https://doi.org/10.2147/CMAR.S340472 DovePress 417 a large number of studies have proved its efficacy.…”
