Acemannan has been previously reported as a direct pulp-capping agent in animal study. This natural material demonstrated its biocompatibility and enhanced reparative dentin formation. The objective of this study was to investigate the action of acemannan as a direct pulp-capping material in human primary teeth with deep caries. Forty-two deeply carious mandibular primary molars from 37 children, aged 7-11 years old diagnosed with reversible pulpitis were studied. After completely removing the infected dentine, teeth with a pinpoint pulpal exposure were randomly divided into two treatment groups: acemannan or calcium hydroxide. A glass-ionomer cement base was applied to all teeth prior to restoration with stainless steel crowns. Clinical and radiographic evaluation was performed 6 months post-treatment. The teeth due to exfoliate were extracted and histopathologically evaluated for inflammation, dentine bridge formation, and soft tissue organization. At 6 months, the overall clinical and radiographic success rates of direct pulp capping with acemannan and calcium hydroxide at 6 months were 72.73 and 70.0 %, respectively. The histopathological results indicated that the acemannan-treated group had significantly better histopathological responses compared with the calcium hydroxide-treated group (p < 0.05). These data suggest acemannan offers a valuable alternative biomaterial for vital pulp therapy in primary teeth.

AIM: The present study was conducted to compare effects of acemannan and formocresol as pulp-dressing agents clinically and radiographically in primary teeth. MATERIALS AND METHODS: Aloe vera gel high molecular weight fractions (AHM) containing native ingredients of polysaccharide (acemannan) and glycoprotein (verectin) were obtained by using the patented hyper dry system after washing out coloured materials with running water from Aloe vera gel slurry. Thirty healthy children aged from four to eight years, they were selected from patients attending outpatient clinic of Pediatric Dentistry department. Each child had at least bilateral deep carious lower primary molar indicated for pulpotomy. Pulpotomy was done in both groups; where group I treated by AHM, acemannan, as a dressing agent while formocresol used as a dressing agent in group II. The study cases were recalled after three, six, nine and twelve months for clinical and radiographic evaluation. RESULTS: The overall clinical success rate of acemannan group was 96.5%, while formocresol group was 89.6%. The two groups were clinically successful with no statistically significant difference between them (p = 0.148). The radiographic success rate of acemannan group was 93.1%, while for formocresol group was 86.2%. There was no statistically significant difference between the two groups (p = 0.385). CONCLUSION: The acemannan showed higher clinical and radiographic success rate compared to formocresol as a pulpulpotomy agent in molars. It can be considered as an acceptable biomaterial for vital pulp therapy of deep caries in primary teeth.

Aim:The present study was conducted to compare effects of biodentine and formocresol as pulp-dressing agents clinically, radiographically and histopatholically in primary teeth. Materials and Methods:Thirty healthy children aged from four to eight years, they were selected from patients attending outpatient clinic of Pediatric Dentistry Department. Each child had at least bilateral deep carious lower primary molar indicated for pulpotomy. Pulpotomy was done in both groups; where group I treated by biodentine while formocresol used in group II. The study cases were recalled after three, six, nine and twelve months for clinical and radiographic evaluation. For histopathological study sixteen lower primary first molars planned for serial extraction were selected, biodentine and formocresol pulpotomies were done, extraction of the treated teeth from each group was done after one and three-month intervals.Results: the overall clinical success rate of biodentine group was 90%, while formocresol group was 80%. The two groups were clinically successful with no statistically significant difference between them (P= 0.278). The radiographic success rate of biodentine group was 86.6%, while for formocresol group was 73.3%. There was no statistically significant difference between the two groups (P = 0.197). Biodentine showed significantly better histopathological results compared to formacresol after three-month interval. Biodentine favored the formation of partial dentine bridge with normal pulpal architecture. The pulp tissue in formocresol group showed necrosis with no evidence of dentine bridge formation Conclusion: the biodentine showed higher clinical, radiographic and histopathologic success rate compared to formocresol as a pulpotomy agent in primary molars. It can be considered as a biomaterial for vital pulp therapy of deep caries in primary teeth.
