To investigate the frequency of angiotensinconverting enzyme (ACE) gene insertion/deletion (I/D) polymorphism genotypes in adults with psoriatic arthritis (PsA), a heterogeneous chronic disease with autoimmune pathology. ACE gene I/D polymorphism inXuences the plasma and tissue levels of ACE and has an involvement in inXammatory mechanism. The frequency of ACE gene I/D polymorphism genotypes was determined in 51 adults with PsA from Kuwait, and compared to that in 100 ethnically matched healthy controls using polymerase chain reaction. The distribution of ACE I/D polymorphism and allele frequencies in PsA patients were not signiWcantly diVerent from controls (P > 0.05). Further analyses of PsA patients showed that ACE I/D gene polymorphism was not associated with family history, clinical manifestations, and disease severity. However, the frequency of II genotype was signiWcantly higher in patients with late disease onset than in those with early onset (25 vs. 3%; P = 0.04). No diVerence was found between the distribution of the ACE genotype in PsA patients and the general population in Kuwait.However, the presence of II genotype may confer susceptibility to the development of late onset PsA.

Psoriatic arthritis (PsA) is a psoriasis-associated inflammatory disease of the joints and enthuses. The occurrence of PsA is linked to the complex interplay of gene environment, and immune system. Genetic factors have long been recognized to play an important role in PsA. Genes within the major histocompatibility complex (MHC) region have been shown to be associated with PsA. These include genes coded in the HLA region, (especially Class I antigens) and non-HLA genes (i.e., MHC class I chain-related antigen A, MICA, and TNF-α genes). Association studies in PsA have also identified a number of genes outside MHC region, including interleukin-1 (IL-1) gene cluster, killer-cell immunoglobulin-like receptors (KIRs), and IL-23R genes. Established systemic treatments for moderate-severe psoriasis and PsA may be potentially dangerous and usually time consuming for the patient and often expensive for the National Health Systems. Tests which could predict which subset of psoriatic patients could develop the most severe forms of the disease (i.e., PsA) or will respond to well-established (UVB irradiation) or other systemic treatments are now required. The goal of genetic test screening is to rapidly and safely identify subjects for preventive or early treatment or extended surveillance prior to the onset of signs and symptoms. Genetic tests today represent a reliable investigation procedure which could rapidly and consistently improve the diagnostic ability of the dermatologist and contribute to the early and correct treatment of the different subsets of PsA.

Backround and Design:Psoriasis is a common inflammatory dermatological disease which may be complicated with joint involvement. It has been suggested that there is an association between HLA-B27 positivity and early onset psoriasis. The purpose of the current study was to investigate the incidence of HLA-B27 positivity in psoriasis patients with arthritis. Materials and Methods: In a total of 96 patients with psoriasis, age of onset, family history, and Psoriasis Area and Severity Index (PASI) values were recorded. The patients were evaluated with regard to physical examination (presence of arthritis), acute phase reactants, HLA-B27 positivity and joint radiographs. Control group comprised of 100 randomly selected healthy individuals. Results: Thirty (31.250%) patients were with psoriasis alone, 66 (68.75%) were with the findings of psoriasis and arthritis. Of the 66 patients, 17 (17.708%) were symptomatic (clinical and radiologic findings) and 49 (51.042%) subjects were asymptomatic (radiologic findings only). Nine patients (6 with psoriasis only and 3 with psoriatic arthritis) and 2 healthy controls were positive for HLA-B27. Conclusion: To carry HLA-B27 antigen increased the risk of psoriasis with an OR of 5.06, and clinically proven psoriatic arthritis with an OR of 10.5 compared to healthy controls. These results need confirmation in a larger group of patients with the inclusion of proper positive and healthy controls. (Turkderm 2014; 48: 131-4)
