Obstructive sleep apnoea (OSA) is common sleep disorder with its multimodal effect on health. OSA, if untreated can lead to various cardiovascular, cerebrovascular, psychological, cognitive as well as sexual morbidities. Continuous positive airway pressure (CPAP) device is considered as the gold standard in the treatment of OSA. Epiglottic collapse (EC) in OSA has evolved an important factor in the management of OSA as the CPAP may worsen the sleep apnoea. EC in itself is classified into primary or secondary, partial or complete, anteroposterior or lateral. Epiglottic collapse is considered as one of the causes for poor adherence to CPAP devices. There are newer diagnostic modalities to diagnose and distinguish primary and secondary epiglottic collapse of which interventional drug induced sleep endoscopy plays a viable and important role. CPAP still serves as a primary treatment modality for multilevel OSA with EC. Surgical treatment modalities for EC is aimed at improving the compliance of CPAP as well as for relieving the obstruction. Surgical options primarily include epiglottectomy, glossoepiglottopexy, epiglottis stiffening operations.

Obstructive Sleep Apnea (OSA) is a prevalent sleep disorder characterized by recurrent upper airway obstructions during sleep, leading to disrupted sleep patterns and potential health risks. This comprehensive review explores various medical and surgical modalities for the treatment of OSA. Continuous Positive Airway Pressure therapy is the standard treatment for moderate to severe cases, but long-term compliance remains a challenge. Oral appliances and novel technologies, such as Oral Pressure Therapy and Upper Airway Stimulation offer non-invasive alternatives. Surgical interventions, like Maxillomandibular Advancement, Uvulopalatopharyngoplasty, Laser-Assisted Uvulopalatoplasty, Radiofrequency Ablation, Genioglossus Muscle Advancement and Tracheostomy, provide tailored solutions for persistent OSA. Maxillomandibular Advancement in particular, has shown significant efficacy in reducing the Apnea-Hypopnea Index. Uvulopalatopharyngoplasty, while widely used, has variable success rates, often necessitating additional therapies. Laser-Assisted Uvulopalatoplasty and Radiofrequency Ablation are minimally invasive options but require careful patient selection. This review highlights the importance of individualized treatment plans, considering OSA severity, patient compliance, and comorbidities. Advances in non-invasive and minimally invasive treatments have expanded options, enhanced patient quality of life and reducing health risks. Tracheostomy, though less common today, remains valuable in specific cases. A multidimensional approach to OSA management is crucial, tailoring treatments to individual patient needs and optimizing overall well-being. The Utility of 3D-printed Cutting Guides in Genioglossus Advancement

Genioglossus advancement is a surgical technique performed to treat obstructive sleep apnea. The genial tubercle, where the genioglossus muscle attaches, is not visible during dissection due to its location on the posterior mandible. Computer-aided design, particularly for the production of cutting guides, has arisen as a modern modification in the procedure to support accurate location of the tubercle. This study aims to demonstrate the utility of cutting guides designed by virtual surgical planning in a series of 7 genioglossus advancement cases. Computed Tomography (CT) data were preoperatively collected and rendered in three-dimension. The surgeon designed patient-specific cutting guides, registered to the occlusion, to ensure the genial tubercle was included in the osteotomy and stayed below the incisor tooth roots. These surgical guides were then 3D-printed. All the virtual surgical planning-designed guides fit appropriately, the genial tubercle was successfully included in the osteotomy in all cases, and no tooth roots were injured. We conclude that occlusal-based 3D printed cutting guides are a useful adjunct to help guide a genioglossus advancement procedure by accurately incorporating the genial tubercle. This provides a safe way to concurrently address correction of obstructive sleep apnea in patients undergoing other procedures.
