Genomic Drivers of Multidrug-Resistant Shigella Affecting Vulnerable Patient Populations in the United States and Abroad

Abstract: Multidrug-resistant (MDR) Shigella infections have been identified globally among men who have sex with men (MSM). The highly drug-resistant phenotype often confounds initial antimicrobial therapy, placing patients at risk for adverse outcomes, the development of more drug-resistant strains, and additional treatment failures. New macrolide-resistant Shigella strains complicate treatment further as azithromycin is a next-in-line antibiotic for MDR strains, and an antibiotic-strain combination confounded by gaps… Show more

“…The Centre for Disease Control (CDC) has categorized Shigella infections as a serious threat because of the increasing trend of drug resistance [ 44 ]. Resistance to ciprofloxacin, β-lactam antibiotics, or plasmid-mediated azithromycin resistance is of high concern [ 45 , 46 ]. Our study observed that over the 20 years, multidrug resistance shigellosis has been increasing in both urban and rural settings, and the rate of this increase is more significant in the urban site.…”

“…Efficacy of the NPs can be increased by tuning sizes, shapes, and chemical compositions of the NPs. Metallic, organic, biomolecular, radio-and antibody modified NPs can effectively distroy bacteria with multiple mechanisms and their potency can be enhanced with addition of ultrasound, magnetic field, light, and ionizing radiation properties of bacteria [8,9], further confirms AMR genomic drivers and HGT.…”

“…However, Shigella spp. have acquired resistance to many antibiotics making treatment more difficult and expensive [7]. Though higher for travellers and children or infants in shigellosis-hyperendemic regions, the risk of shigellosis is also increasing in industrialized countries.…”
