Germinal Center Cell‐Derived Lymphomas of the Skin

Abstract: Germinal center cell-derived (GCC) malignant lymphomas are B-cell tumors that represent neoplastic equivalents of normal lymphoid follicles. These lymphomas include, in the Kiel classification, three main types: centrocytic lymphoma, centroblastic-centrocytic lymphoma, and centroblastic lymphoma. We have diagnosed 13 cases of germinal center cell-derived cutaneous lymphomas among 178 evaluated malignant lymphomas of the skin. Some clinical, histologic, and immunologic aspects are discussed in this article.

“…cutaneous di¡use large B cell lymphomas of the leg. The speci¢c gene expression signatures found in FL and primary cutaneous di¡use large B cell lymphomas, not involving the leg, give further support for a GC origin of these entities, which was shown previously (Kerl and Kresbach, 1984;Gellrich et al, 2001) and may explain the overall favorable prognosis of these patients. A larger set of tumor samples may help to identify single or multiple genes, which will be of diagnostic, prognostic, and therapeutic value, and will intensify our understanding of the diverse biology of these tumors.…”

“…Differentiation of CLH from MZL using only hematoxylin and eosin‐stained sections remains difficult, however, and additional studies are usually employed in an attempt to demonstrate B‐cell predominance (40% of MZL cases have a B:T‐cell ratio of ≥ 3:1) 15 and a monoclonal B‐cell population. Primary cutaneous follicular center cell lymphoma, 23–26 especially when it demonstrates a follicular (multinodular) growth pattern, 26 can also closely mimic CLH.…”

“…A lymphoma is considered to be "primary" to the skin if there is no extra-cutaneous disease at the time (and within the first 6 months) of its diagnosis (2,4,5). Primary cutaneous FCL has been reported in the European Organization for Research and Treatment of Cancer (EORTC) for Cutaneous Lymphomas (2) and in other studies as a distinct entity, which has a good prognosis and rarely disseminates to extracutaneous sites (4,6,7). But it has been argued, that the entity classified as cutaneous FCL in the EORTC is not the same as those described in other classifications (5,8).…”
