Background: A rare complication of diverticulosis of the colon is giant colonic diverticula (GCD). The condition was first described in English literature in 1953 [1, 2]. Methods: A Medline search was undertaken for English, French and German language articles on ’giant colonic diverticula’. Results: A total of 135 patients were identified, presenting with a total of 155 GCD [1–98]. With a complication rate of 28% and an operative mortality of 5%, GCD seems to have a high clinical significance. Radiological examination of choice seems to be plain abdominal X-ray and CT examination, barium enema carries the risk of perforation of the diverticulum, and should not be performed. Conclusions: There are different therapeutic options, in our opinion diverticulectomy alone is not the treatment of choice. Because of the possibility of recurrence and oncological reasons, colectomy seems to be the best treatment. The creation of a protecting colostomy depends on other operative findings.

“…Note is made of the example illustrated in Case 2 and figure 4, where the abnormality occurs in a redundant sigmoid colon and therefore is found in a location which is not classically thought of as being related to the sigmoid colon. Consequently, based on history and examination alone, the differential diagnosis includes many varying entities such as duodenal diverticulum, Meckel's diverticulum, duplication cyst, sigmoid volvulus, and emphysematous cholecystitis [7]. As the ostia are often are not directly visualized, endoscopy is not of benefit [8].…”

IntroductionThis case series illustrates the clinical presentation and radiological findings of two patients in whom the diagnosis of a Giant Colonic Diverticulum (GCD) was histologically confirmed. We also discuss the pathogenesis and differential diagnosis.Case presentationIn one case, the patient had no previous large bowel symptoms and intra operatively no other colonic diverticula were found. Both were treated with surgical resection.ConclusionThe preoperative diagnosis of GCD is made radiographically, and the importance of the early recognition of radiological findings, especially in asymptomatic disease, is emphasised.

Imaging Diagnostics of Colonic Diverticulosis and Its Complications Radiodiagnostic imaging modalities in diverticulosis and diverticulitis for visualization of complications include supine radiograph of the abdomen, ultrasonography, barium studies of the colon, in case of diverticulitis and suspicion of penetration only water-soluble contrast medium studies, spiral-CT and magnetic resonance imaging (MRI). In cases of endoscopic failure to localize diverticular bleedings because of massive intraluminal blood clots, selective and superselective angiography is indicated for localization of the bleeding source and, if necessary, transcatheter embolization or vasopressin infusion to stop the bleeding. In endoscopical failure to pass the stenosis of inflammatory origin because of diverticulitis, only water-soluble contrast medium studies are indicated, mainly to visualize the post-stenotic areas of the colon orally to the stenosis. In cases of penetration or suspected perforation, fistulas in surrounding organ structures like adjacent bowel loops, urinary bladder or muscle tissue, ultrasound or spiral-CT are the favorite imaging modalities. Up to now, MRI does not play the role as ultrasound or spiral-CT do.
