Giant ileal inflammatory fibroid polyp causing small bowel obstruction: a case report and review of the literature

Abstract: Introduction: There are several types of small bowel pathology that can lead to small bowel obstruction or intussusception. The etiology causing small bowel obstruction varies by age. Benign disease is the typical cause in children and adolescents while malignant or adhesive disease is far more common in older patients. Although cases of adult intussusception caused by benign processes are rare, there are reports of inflammatory fibroid polyps causing adult intussusception of the terminal ileum published in th… Show more

“…7 Most other case reports document only solitary IFPs, including solitary giant IFPs. 8 Recurrence of multiple polyps has not been encountered up to now. We report a rare case of a young female patient who underwent three laparotomies for recurrent intestinal obstruction due to multiple IFPs.…”

“…The occurrence of IFPs in the small bowel is rare, accounting for only 18% of all cases, and it is less frequent in the ileum, where these polyps rarely cause intussusceptions [4]. Recent review of cases of ileo-ileal intussusceptions caused by IFPs published in English, found only eight cases [7].…”

“…The aetiology remains unknown, but it could be a consequence of chronic irritation and inflammation or a consequence of extreme reaction of the body to an intestinal trauma. Histologically, they arise from the submucosa and are characterized by vascular and fibroblast proliferations and an inflammatory response, dominated by eosinophils [7,8].…”

“…Patients with small bowel inflammatory polyps are typically asymptomatic until the onset of complications such as small bowel obstruction due to intussusception or bleeding 3,6. Most case reports of inflammatory polyps are complicated cases with intussusceptions or bleeding that were diagnosed and treated by surgical resection 1,2,4,5,7…”
