Giant intradiploic arachnoid cyst for 13 years

Abstract: A case of intradiploic arachnoid cyst is reported. The patient presented with a progressively enlarging swelling situated over left frontal region for approximately 13-years following blunt trauma to head. Magnetic resonance imaging showed an intradiploic fluid containing cyst having intensity like cerebrospinal fluid (CSF). He underwent craniotomy and successful surgical repair. Intraoperatively CSF cyst was located in the frontal pole with a large defect over inner table and large rent in the dura. It was li… Show more

“…Surgical management is the mainstay of treatment of PTIDLCs and indications for surgery include large disfiguring swelling and persistent headache [ 7 , 15 , 16 ]. The surgical procedure involves watertight dural closure using duraplasty followed by cranioplasty using autologous split calvarial graft and carries a good prognosis.…”

“…Механизм возникновения арахноидальной кисты черепа изучается. В ряде случае удается установить связь с черепно-мозговой травмой, обычно в детском возрасте [14]. В таком случае лоскут арахноидальной оболочки попадает в диплоэ кости через посттравматические дефекты ТМО и внутренней кортикальной пластинки черепа.…”

“…The term “intradiploic arachnoid cyst” was first described by Weinand et al [ 2 ], when cranial arachnoid cysts developed as folded extensions of the arachnoid membrane through minor defects in the dura mater, eroded through the inner table, expanded within the diploe, and eroded the outer table of the skull [ 2 ]. Cerebrospinal fluid accumulation within the diploic space is extremely rare [ 3 ]. Intradiploic cerebrospinal fluid (CSF) collections can occur due to trauma but can also be non-traumatic.…”

“…Rarely, these cysts may also appear when there is no trauma in the patient's history. On the other hand, generally, intradiploic arachnoid cysts, or leptomeningeal cysts, occur after a head trauma or a cranial surgical intervention [ 3 ]. In the literature, these cysts are called “intradiploic CSF fistula”, “intraosseous leptomeningeal cysts”, and traumatic or post-traumatic “intradiploic arachnoid cysts”.…”
