Giant subungual glomus tumor: clinical, dermoscopy, imagiologic and surgery details

Abstract: Glomus tumor is a rare benign vascular hamartoma derived from the modified smooth muscle cells of the glomus body. Paroxysmal pain is the leading symptom. A 78-year-old woman presented at our clinic with a 50-year history of a slow growing painful purple blue tender nodule under the right fourth finger nail. Dermoscopy and magnetic ressonance confirmed the presence of a large subungual tumor and total surgical excision was performed after partial nail plate avulsion. Histologic examination confirmed a glomus t… Show more

“…27 In describing the dermoscopic features of the glomus tumor, a wide variety of terms have been used in the literature. 9,10,[16][17][18][19][20][21] The primary purpose of our study is to investigate whether there is any finding of glomus tumor that cannot be detected by clinical examination but can be detected by dermoscopic examination.…”

“…15 The dermoscopic features of the glomus tumor have been described in a few case reports and series. 10,[16][17][18][19] In describing the dermoscopic features of the glomus tumor, the terms such as "structureless bluish area and linear irregular vessels," 16 "erythematous vascular area," 16 "pinkish bluish," 17 "structureless bluish patches mixed with linear irregular vessels," 18 "lamellar scaling of the nail plate," 18 "subungual linear and pink-whitish structure," 9 "localized structureless erythema," 10 "candy-cane appearance" (longitudinal red bands interspersed with longitudinal white bands), 10 "discreet linear vascular structures," 19 "deep red-purple discolored area with blurred borders," 20 "a band of longitudinal erythronychia," 20 "small red ramified vessels," 20 and "pinkish structureless pattern with arborizing vessels" 21 were used in the literature. The aim of this study is to determine the clinical and dermoscopic features of the cases in a subungual glomus tumor series and to investigate whether there is any finding of glomus tumor that cannot be detected by clinical examination but can be detected by dermoscopic examination.…”

“…18,19 Nevertheless, there is no consensus on the most suitable term to describe these vessels. In this sense, terms such as 'ramified vessels', 12 'prominent telangiectatic vessels', 5 or 'linear irregular vessels' 13,14 have been used. In our experience, these vessels are mostly linear, moderately thin, with multiple bends and scarcely branched.…”

“…[11][12][13] However, current data regarding dermoscopy of GTs are limited and are mostly based on isolated reports and two relatively small case series. 5,7,12,[14][15][16] The aim of this study was to analyse the dermoscopic features and patterns of a large series of SUGTs and EUGTs.…”
