Glasgow coma scale pupil score (GCS-P) and the hospital mortality in severe traumatic brain injury: analysis of 1,066 Brazilian patients

Abstract: Background Pupil reactivity and the Glasgow Coma Scale (GCS) score are the most clinically relevant information to predict the survival of traumatic brain injury (TBI) patients. Objective We evaluated the accuracy of the GCS-Pupil score (GCS-P) as a prognostic index to predict hospital mortality in Brazilian patients with severe TBI and compare it with a model combining GCS and pupil response with additional clinical and radiological prognostic factors. Methods Data from 1,066 patients with… Show more

“…The outcome of traumatic brain injury (TBI) is influenced by a combination of factors, some of which can be modified and others that cannot 24 . These factors include age, sex, level of education, cause of injury, level of injury severity, pre-existing health conditions, time to receive definitive care, and the quality of clinical treatment 1,18,19,25,27,28 . Identifying the key variables associated with TBI outcomes is crucial in reducing mortality and morbidity.…”

“…It is a simple scoring system that can be readily applied in clinical practice by combining GCS with an assessment of the reactivity of a patient’s pupils and provides an extended index of clinical severity superior to the GCS alone because it adds additional information on pupil reactivity. Its performance is comparable to more complex algorithms such as IMPACT and CRASH 8910. Serial assessment of GCS Pupils score can be used to provide trends in a patient’s condition.…”
